Sample Achievement Tracking Log

This document is a tool to help you track your work throughout the year, identify your role in the success of the task or project, articulate the impact of your
work, and report your achievements in each performance review cycle.

Tips:
 Use your job description and your annual goals as a way to frame your work. Use your calendar to look back at past events or projects.
 Focus on and take pride in your own individual accomplishments. Whether you worked as a team or on a part of a larger project, you had an impact.
Identify what you did and be able to explain your individual work.
