AFFIDAVIT OF UNDERTAKING

I, _________________________, of legal age, married, Filipino and a resident of Davao

City, Philippines, after having been duly sworn to in accordance with law do hereby depose and
say that:

1. I am the same person who caused the preparation of this document;

2. I am currently an employee of the ______________________;

3. The status of my employment with the University is PROBATIONARY since I lack

the educational requirement of a teacher’s license for my regularization as provided
for by law;

4. I am aware of the fact that the absence of my teacher’s license is a ground for my
termination as a full-time faculty.

5. I acknowledge the fact that my continued employment with the School as a full- time
probationary employee was extended to me on humanitarian consideration on the part
of the School.

6. I undertake and commit to earning my teacher’s license by the end of

_______________ as a pre-condition to my continuing employment with the
University;

7. I undertake and commit to improving on my punctuality and pass the over-all

evaluation of my performance.

8. I understand and agree that failure to fulfill my undertaking and commitment as

mentioned in the preceding item shall be a valid and sufficient ground for the
cessation of my employment with the School;

9. I am executing this affidavit to attest to the truthfulness and veracity of the preceding
statements and to inform the proper authorities concerned of the matter, and for such
other legal purpose/s this may serve.
