By virtue of my fiduciary trust, and in consideration of your acceptance of my offer, I hereby


give you the authority to negotiate the sale of my property with a commission of ____________

I hereby certify that I am the owner in fee simple of said property, free and clear from any liens
and encumbrances other than those annotated in the original copy of the title on file with the
proper registry of deeds.

upon agreement.
Should deposit money paid on account of purchase is hereby forfeited by the purchaser, the
amount equal to or proportionate to the percentage of his commission shall be retained by the
broker.

commission provided the name of the prospective buyer has been registered with me during the
period of your authority.
