TRIP TECHNICAL REPORT

Fault Type: 4.16 kV Incomers Forced to Open Type of Relays: N/A

Brief of the failure :

PRC-Steam Generation Area Operation Team announced that a power interruption occurred for some the
LV loads , feeding from Substation#13 , .

PSD-POS immediately mobilized to the substation#13 and observed the following with ABB team :-
1. Both 4.15 kV Incomers were opened at 9:53 am .
2. Lost of Power in the main LV switchgear “480 V PC13.1 ” .
3. Non of 4.16 kV Motor Pumps or Compressors were in service or running mode before the power
interruption .

PSD-POS discussed with ABB site team and concluded to restore back the power supply after confirming
that the Circuit Breakers were not opened due to protection alarms or trips . The system was successfully
normalized at 10:03 am .

Brief of the running activity :

ABB was performing the Protection Relay Replacement of 4.16 kV Bus Tie-1 in MS13.1 as part of
Refinery Substations Upgrade Phase - 1 (Stage-2) .The activity was almost completed and the site
technician was dressing the wires and fixing the cable ties in the cable raceways .

Expected reasons and observations :

The main DC power supply Terminal Blocks are equipped in Bus Tie-1 cubicle and distribute the DC for
all feeders in MS13.1 (3 Buses ) . ABB noticed that the main DC cables were loose in TB connections
,which will cause fluctuation in DC supply and will effect some of Auxiliary relays and contactors to
chatter while shaking the main DC cables in cable raceways . It was observed that the existing main DC
cables are not terminated in proper way .

Performed Actions :

ABB immediately informed the project ESO for the observation and took the permission to do the tightness
in the main DC TBs with proper insulated tools as it is not possible to switch off the main DC of MS13.1 .

Note :- ABB team has a practice to do tightness for all TBs for any isolated feeder before energization .
However , the above mentioned main DC TBs were Live and ABB could not worked or disturbed these TB
as might effect the DC for other MV feeders .
