ALS Assessment Form 2

RECORD OF MODULE USE AND MONITORING OF LEARNER’S PROGRESS


This form is an assessment record. The learner records each learning module
he/she completed, the learning strand to which the learning module belongs, the date
of accomplishment, the results of the pre-test and post-test, and other related activities
accomplished. The learner is asked to reflect on his/her performance, identify personal
strengths and weaknesses, and suggest ways to overcome difficulties that may have
been encountered. The ALS Teacher will also evaluate the learner’s performance and
provide feedback on the learner’s level of mastery of the module content.

This form can be accomplished in Mother Tongue, in Filipino or in English.

The ALS teacher may assist the learner or complete this form if the latter is not
confident to do so during the conduct of the sessions. This form should be reviewed
and updated at least once a month or as deemed necessary.

The information will enable the ALS teacher to help the learners develop their
skill in self-directed learning. This form shall be used as reference document when
reviewing the Individual Learning Agreement (ALS Assessment Form 1).

The form should be reviewed by the ALS teacher and the learner each time a
new set of learning module is issued. The ALS teacher should discuss the learner’s
progress and suggest ways of improvement.

The form should be included in the working portfolio (work folder).

It should also be included as one of the selected items for the presentation
portfolio if/when the learner decides to have the portfolio assessed as part of the
expanded A&E certification system.

How to Use this Form

In this form the learner records each learning module completed, the learning
strand to which the learning module belongs, the date of accomplishment, the results
of the pre-test and post-test, and other activities the learner has completed. The
learner is also required to assess his/her performance by identifying personal
strengths and weaknesses, and suggest ways to overcome such. This record can be
written in Mother Tongue, in Filipino or in English.

For each completed learning module, the learner with the assistance of the ALS
Teacher should write the following:
a. Title of the learning module
b. Learning strand to which the module belongs
c. Date when the learner started and finished accomplishing the module
d. Result from the learning module, “Let’s See What You Already Know”
(pre-test)
e.g., total number of correct answers = 7
total number of items 10
Result from the learning module, “What Have You Learned” (post-test)
e.g., total number of correct answers = 15
total number of items 20
e. Other Related Activities Accomplished column to supplement the
learning (e.g. reading materials, experiments)
f. Learner’s Self-Assessment of Performance. The learners are asked to
write down what they enjoyed, did not enjoy, difficulties they
encountered, and the possible ways of solving these problems in the
next module.
g. ALS Teacher’s Assessment. The ALS teacher should assess also if the
learner mastered or did not master the contents of a specific module. As
a guide, if the learner got an average percentage of 75 and above, the
learner mastered the lesson. If the learner got an average percentage of
74 and below, the learner did not fully understand the module contents
and would benefit from individual remediation lessons from the ALS
Teacher.
h. ALS Teacher’s Remarks. The ALS Teacher will provide remarks on the
learner’s performance after accomplishing the module.
The learner and the ALS Teacher should affix their signatures at the bottom of
the form with the specific date the form was accomplished.
