Subject: MODIFICATION / AMENDMENT AGAINST THE NAME OF SUB-DEPARTMENT.

Respected Sir,

With profound respect I would like to drawn your kind attention towards the name of Sub-department
appearing in my monthly salary slips. Sir, I have now been working in CCO since 04.12.2020 as Assistant
Engineer Operation but the name of Sub-department in my salary slip is appearing as “Chemical
Operations”.

I therefore, request to your gracious honour that the name of Sub-Department may kindly be got corrected /
changed in my salary slips as CCO instead of Chemical Operations, in order to avoid any complications at
belated stage, please.
