In order to make this vision a reality I will focus deeply on my studies this year in the PHSU program as these are the courses I
will require in order to apply for University. The foundation of my vision is based on my success in this program. After PHSU,
I plan on applying for the Nursing program at Ontario Tech University.

The actions I will take to make sure I am successful in this years studies will be to prioritize my assignments and organize
them with my personal/daily life. I have mapped out all of my due dates for my courses as well as set time aside for work, and
family time. To ensure the upmost success, I need to make sure I don’t overwhelm myself. I tend to overload myself to the
point of burnout. By organizing and planning everything out I will minimize the stress and risk of overwhelming myself.
If I go further into the future and say I am successful enough to get into the Nursing program at Ontario Tech University, I will
continue to follow the actions I listed previously in order to successfully graduate from the BSN program.

GOALS & PRIORITIES

1.My goal is to start going to bed at a reasonable time to be well rested for the day. Currently I am going to bed at early hours
of the morning (around 2-3am) and waking up early to go to work (between 6-7am). I will implement a new sleeping schedule,
by turning off all electronics by 10:30pm in order to help me get into a rested state and be able to fall asleep by 11pm. I will
have a list of different activities or things that may help me fall asleep faster. For example, drinking tea, reading or even taking
melatonin (as needed). Having a better night’s rest will help in preventing burnout and will increase my amount of energy and
focus when it comes to completing academic tasks and daily activities.

2.My goal is to lay out all of my deadlines, and work schedule in order to organize and plan out my time. Currently I have due
dates written out in an agenda, however I do not have dedicated time to work on specific assignments, nor do I have them
working around my work schedule. I can easily become overwhelmed and will then begin to leave my tasks and assignments
to the last minute. I will implement the maps created in part A of the mapping assignment, and use that to properly divide my
time. I will set a specific amount of time aside for each assignment days in advance and ensure that my work schedule is
included. For example, if I have a small quiz for computers and a large assignment for prep, I will designate two times
throughout the week to work on the assignment for prep, whereas the quiz I may do after I get home from a shift.

3. My goal is to start living a heathier lifestyle when it comes to my mental and physical health. I am currently at a healthy
weight for my height and age, however my patterns when it comes to eating, sleeping and exercise are irregular and unhealthy.
I will implement a tracking method to watch what I am eating and how much I exercise on a weekly basis. Once I get into a
routine it will be easier for me to continue with a healthy pattern. Doing this will help me feel more confident and promote my
mental health, which in return will keep me focused on my deadlines and obligations.

MOTIVATION & REWARDS

1. I will reward myself by purchasing a new duvet cover for my bed. This will keep me motivated as I have been interested in
purchasing a new one for a while however, have continued to talk myself out of it. If I make this as a reward for fixing my
sleep schedule, it will give me more reason to really focus in on doing so. Also, a new duvet cover will help in the sense of
wanting to sleep because my bed is more comfortable and I will want to spend more time there.

2.I will reward myself by taking a day off and setting a time to go for a date with my boyfriend. This will keep me motivated Meaningful and

complete that goal and then by that time will be in a routine and will be able to continue on that healthy path.

1.One roadblock that may affect my ability to make certain deadlines and meet obligations is my work schedule. I currently
work two jobs, which averages out to 4-6 days a week. This may affect my ability to be successful with deadlines and How to mitigate or

1.The first person that is supporting me is my boyfriend. My boyfriend encourages me every day to work hard for my goals
and boosts my confidence by expressing to me how he sees me and what he thinks I can accomplish.

2.The second person/people that are supporting me are my parents. They have supported me, giving me a home and helping
me pay for my education. They have supported me through my first time going through post-secondary school and are now
again supporting me and encouraging me to follow my dreams and passions. They will always be the ones to back up my ideas
and aspirations no matter how many times I change my mind.

3.The las person that is supporting me is my grandmother. She supports me in a different way as she keeps me motivated and

is what I aspire to be. My grandmother used to be an RN at Lakeridge Health Hospital for many years and retired from there.
My grandma tells me various stories from her career and makes me want to push harder to accomplish my goals.

Academic Pathways
College is a great jumping off point for continued post-secondary education (college or university). You have a lot of
pathways, bridging programs, credit transfers, application options available to you. Can you research some of these options

What is Plan A?
We’re supposed to be talking about our plans A & B, but if I’m being completely honest, I have been all over the place
with my plans and ideas for careers and futures. Originally, I had gone to post secondary school with the intention of
becoming either a police officer or correctional officer. I had completed my Community and Justice Services diploma at

semester I will be able to gain the knowledge and understanding in order to guide me through to Nursing. After the
completion of the PHSU certificate, I plan to apply to Ontario Tech for the BSN program and begin in September 2022.

What is Plan B?
My plan B is similar to my plan A. If for whatever reason I’m not accepted into the BSN program at Ontario Tech
University, I’ve been thinking about applying to a college level program that is still in the medical field. I have been
deeply considering the Paramedic program as a back up plan. I’ve been thinking about this as I already have tried physical
testing needed for first responders as well as this will still keep me in the realm of medicine. I’m not set on my plan B as of
yet because I like to keep my options open and see how things go.
