Element

1.0 Context:

Any work or movement near a drop of 1.8meters or more is considered a high-risk activity as falls are a
common cause of work related deaths. Numerous work activities conducted at KPC are performed at height. It is
therefore necessary to establish strict controls in order to remove the potential for fatal incidents resulting from
falls.

2.0 Purpose:

To ensure that adequate fall protection and prevention measures are implemented to safeguard
personnel from falls that could result in fatal injuries.

3.0 Scope:

This element applies to any task or movement near a vertical drop of 1.8 meters or greater. This includes
the provision for the design, construction, maintenance, and removal of temporary work platforms, and structures,
within KPC mining and infrastructure leases.

4.0 Element Requirements Accountability

4.1 Hazard Identification Risk Assessment and Control


A survey must be undertaken in order to identify all tasks where there is the potential to fall Divisional
from a height greater than 1.8 meters. The survey must record: General Manager

 location of the task
 type of fall prevention or protection measures used
A Job Safety Analysis (JSA) must be conducted prior to any work being performed above a Department
height of 1.8 meters and/or at any time the scope of work changes which may increase the Manager
risk of a fall.
Any task where the potential for falling cannot be eliminated using fall prevention Department
measures then fall protection equipment must be provided. Manager

4.2 Selection, Training, Competency and Authorisation


All personnel conducting JSA’s for working at heights must: General Manager

 have completed basic working at heights awareness training


 be assessed as competent to perform the JSA
The training and competency assessment system shall ensure: General Manager

 competency standards for personnel required to
 select, use, inspect and maintain fall protection equipment
 erect, inspect and dismantle scaffolding
 operate elevated work platforms, scissor lifts or man lifts
 perform maintenance on elevated work platforms i.e. scissor lifts, power steps,
cherry pickers
 training and competency assessments for the above personnel
 the recording of all training undertaken and results of competency assessments
 regular re-assessments of competencies and retraining when required
An overview of KPC working at heights safety requirements must be provided as part General Manager
of the general induction program. HRD

Rescue personnel must be suitably trained in the emergency response requiring the Manager Safety

4.3 Communication and Awareness


Awareness information and/or instruction shall be provided periodically to personnel who Department

 fall prevention and protection measures


 safety requirements for scaffolding, elevated work platforms and ladders
 emergency control requirements
 inspection and maintenance of fall protection equipment and anchor points
Changes to working at heights requirements must be: Department

 communicated to personnel who may be affected
Where work at heights is being carried out, information must be provided on the hazards Department
present and safety requirements. This may be achieved by: Superintendents
 placing warning signs and barricades
 signage displaying fall protection requirements
 verbal methods (pre-start talks etc)

4.4 Design, Purchase, Fabrication, Installation and Commissioning
Purchase specifications must be developed for all working at heights equipment Manager Safety
including :
 fall prevention equipment (Refer glossary of terms)
 fall protection equipment (Refer glossary of terms)
Design specifications must be developed for working at heights equipment or controls Manager Safety
including:
 fabricated work platforms
 fixed ladders
 man cages
 handrails on mobile equipment
A system shall be in place to ensure the commissioning of elevated work platforms prior to Manager Safety
use at KPC.
Fall prevention equipment must consider: Manager Safety
 Kep/Men/555 – Decree of mines article 95
Anchorage points must be fitted by a competent person to ensure that they are Department
positioned appropriately and can take the required load. Manager

4.5 Work Method and Condition Control


Fall prevention measures must be the preferred method to control risk where work is Department
performed above a height of 1.8 m, where this is not possible, fall protection measures must Manager
be applied.
Only persons with the appropriate KIMPER shall operate elevated work platforms. Department

Access to scaffolding shall be controlled by the use of the SCAFFTAG system. No work shall Department

Where safety harnesses are used, they must be attached to an anchor point at all times. Department

4.6 Maintenance
A maintenance program for all elevated work platforms must include: Divisional

 schedules for planned maintenance
 personnel trained and deemed competent to perform the maintenance
 maintenance and repair work undertaken in accordance with the manufacturer
specifications and operational experience
 the use of manufacturers or approved compatible parts
 the recording of all maintenance and repair work conducted
There must be a program to ensure that all Fall Protection equipment is: General Manager

 registered and numbered


 maintained and stored to manufacturers specifications

4.7 Emergency Controls


A rescue plan for personnel using Fall Protection equipment must be in place prior to any work Department
being undertaken. Manager

4.8 Monitoring, Inspections and Audits


Annual audits shall be performed to verify the quality and effectiveness of the requirements General Manager
set out in this element. As a minimum this will include :
 schedules for audits
 audit verification of:
 quality of the JSA’s performed
 quality of the maintenance program
 compliance to training, competency, and awareness requirements
 compliance to design, purchase and commissioning requirements
 communication and reporting requirements
Inspection of all fall prevention and fall protection equipment must include: General Manager

 schedules for conducting inspections


 requirements for inspections to be undertaken in accordance with manufacturer
specifications and operational experience
 the recording of all inspections conducted
 inspections being carried out by a competent person
Fall protection equipment must be: Department
Manager
 inspected and certified for use on a six monthly basis
 inspected by the user before use
Scaffolding and components must be inspected and recorded using the SCAFFTAG located Department
on the scaffold: Manager

 before erection
 prior to initial use
 weekly while erected
 prior to use after inclement weather
 whenever modifications are made to the scaffold
Anchorage points must be inspected by a competent person to ensure it can take the required Department
load. Manager
Task observation on all work at heights activities must be undertaken to ensure Department
requirements are being applied correctly: Manager

4.9 Reporting, Assessment and Corrective Actions


Hazards or defects identified with working at heights equipment must be reported, Divisional
assessed, and must include : General Manager

 actions required to stand down or destroy unsafe equipment (safety harness, ladder
or scaffolding)
 the recording of actions taken to correct the hazard or defect
Fall protection equipment must be destroyed following a fall or where inspection has Department
shown evidence of excessive wear or mechanical malfunction. Manager

5.0 Glossary of Terms

Accountability The person designated to ensure that the assigned requirement is implemented. This
person is held liable and answerable to the level of compliance against the stated
requirement.
Anchorage Point Secure points of attachment for lifelines and lanyards. The anchorage must be able
to withstand a load of at least 15kN for a single person anchor point.
At Height Determined as 1.8 meters or more and applies to an area where it is possible for a
person to fall 1.8 meters or more.
Authorised Permission granted by the relevant Manager or nominee to carry out specific tasks.

Competent Person A person having a combination of training, education, experience, acquired


knowledge and skills enabling them with the capability to perform specified tasks to a pre-
determined quality which may be assessed against defined criteria.
Contractor A firm or person employed under a contract to provide services to PT Kaltim Prima
Coal as distinct from a contract of employment.
Department Manager The person nominated by KPC as Manager under the organizational structure,
also the most senior position with a firm contracting work to KPC.
Fall Prevention Systems that incorporate mechanical assistance and/or barriers designed to prevent
falling whilst working at heights. These may include:
 elevated work platforms
 scaffolds
 fabricated work platforms
 ladders – portable and fixed
 man cages
Fall Protection Is a system to arrest their falling, and includes:
 safety harnesses
 lanyards
 retractable reels – static lines
 anchorage points
Must / Shall Must / shall is to be interpreted as mandatory.
Operational A person with a minimum of three years experience relating to the purchasing,

competent.
Potential to cause a A hazard that has been identified that if not properly controlled will contribute to a
Fatality / Risk of a fatal incident. This can be derived from previous fatal incident experience and /or
fatality having being identified by means of a risk assessment as having a consequence for
fatal injuries.
Risk Assessment A risk assessment is the estimation of the level of risk associated with and identified
hazard to determine the resources needed to control it. Risk is measured in the terms
of consequence and probability.
Safety Harness Is a restraining device worn for the purpose of stopping the wearer from falling to
the floor and/or enabling retrieval of a person from a confined space.

Scaffold A temporary construction which consists of a framework and an elevated


working platform for supporting personnel and materials.
