Interim Guidelines on the Preparation, Submission, and Checking of School Forms
for the School Year 2020-2021

Frequently Asked Questions (FAQs)

1. Will the same guidelines be applied for the next school year?
- No. DepEd Memorandum 14, s. 2021 shall only be applicable for SY 2020-2021,
unless otherwise the suspension of face to face classes is extended .

2. What is the age to be recorded in SF 1?


- The age to be recorded in the form is the learner’s age by October 31, 2020.

3. Do I need to backtrack my record of attendance of learners and revise my SF 2 for the


first and second quarters?
- No. The SF2 that are already prepared by teachers for the first two quarters
SHOULD be adopted without any further modification. The adoption of LACT is
only applicable and should only be used for the third and fourth quarters of SY
2020-2021.
- For those who did not prepare SF2 prior to the issuance of LACT guidelines,
teachers may adopt the LACT guidelines in the preparation of SF2 for those
reporting months.

4. What is the reporting cut-off date for enrollment count?


- Reporting cut-off date for enrollment count is as of October 9, 2020.

5. Who will choose the applicable LACT?


- Teachers/advisers have the freedom to choose the applicable LACT in checking
the learner’s attendance. Refer to Annex 2 of the Memorandum for the list of
LACTs that can be used.

6. Which LACT should I choose?


- It depends on your adopted distance learning modality. Refer to Annex 2 of the
Memorandum for the list of LACTs that can be used.

7. Can I use different LACTs in my classes?


- Yes, as long as the chosen LACT is applicable to your classes. Refer to Annex 2 of
the Memorandum to see which LACT can be applied to your classes.

8. Is it possible that for one section, each learner has a different LACT?
- Yes. Learners in one section may have different LACTs.

9. What if there are multiple modalities being held in a section or a class, does the teacher
need to prepare separate SF2 for each modality?
- If this is the most convenient and useful to the class adviser, he/she may do so.

10. In LACT 1, is the student considered present if he/she is only able to enter the online
learning platform and attend the class for a few minutes due to internet connectivity
issues?
- Yes, as long as there is communication made by the learner/parent/guardian to
the teacher that the learner is having internet connectivity issues.

11. In LACT 2, should a learner still be considered present even in the delayed or
non-submission of a module?
- Yes, as long as there is communication made between the parent/guardian or
learner and the teacher.
12. How can we record the attendance of learners using the 6-class day schedule if the SF 2
is only designed for a 5-class day schedule?
- DepEd ICTS is in the process of updating our school forms in the LIS but feel free
to manually edit the school form on your own, for now, to follow the prescribed
guidelines of DM 14, s. 2021.

13. When should a student be dropped out of class?


- Teachers are highly encouraged to exercise due consideration and reach out
to learners.
- A learner can only be considered drop-out or no longer participating in
learning activities (NLPA) if he/she and his/her parent voluntarily
withdraw from schooling or if he/she incurs seven weeks of
non-attendance in his/her online classes and/or non-submission of
modules/activity sheets without any communication/contact efforts being
made by the learner or parent/guardian with the teacher.

14. What communication efforts are considered valid as a form of attendance and/or
communication?
- Contact/communication efforts may vary depending on the distance learning
modality and may include but not limited to electronic/digital communication,
using of designated drop/pick up kiosk/middleman for printed modules, phone
calls, text messaging, handheld CB radio, letter correspondence or attending
classes/meetings, and/or physical appearance of parent/guardian and adviser
within the parameter of the existing local/national health safety protocols of
IATF for COVID-19.

15. Do I still need to accomplish SF 3?


- If there is an existing form (electronic/printed) being used to monitor
distribution of learning materials or modules, there is no need to make another
recording of the same data in SF3.

16. Are there any amendments to SF 7?


- None. The same form shall be accomplished for SY 2020-2021.

17. How can I update SF 8 if I am not able to meet the learners in-person to measure their
BMI?
- SF 8 is not mandatory but parents or guardians may administer the actual
measuring of weight and height (Body Mass Index) and just submit a reading
result to the class adviser.

18. Am I required to update SF 8?


- Generally, no. The BMI feature in LIS is temporarily relaxed and does not require
users to update BMI data.
- However, for those grade levels in elementary that are beneficiary of health and
nutrition support programs, the BMI data may be deemed relevant.
- Teachers are highly encouraged to collect BMI data provided that the health and
safety protocols are being observed.

19. What if I already filled out the portions of core values and character traits in SF 9, do I
still need to revise or redo it?
- No, you can leave the filled out core values and character traits as is.

20. Where can we get a copy of the revised school forms?


- DepEd ICTS is in the process of revising and updating the school forms. Please
wait for the forms to be uploaded in the LIS. Nevertheless, you can manually edit
the forms based on the prescribed guidelines as you wish.
21. Is the internal Division Level Checking of School Forms allowed?
- No. The annual checking of school forms shall only be done at the school level by
the School Checking Committee.

22. Is the result of the checking of forms conducted by the School Checking Committee valid
and final?
- Yes. It can also be used by the school to complete its submission in LIS.
