GRADUATION REQUIREMENTS

All students will participate in a graduation audit at the beginning of the semester. Students are
required to submit the graduation audit to ensure the student meets FNP Program
Requirements. Students must score a 100 to be eligible for graduation. Students who have not
currently met FNP Program Requirements will be notified of missing items in the assignment
drop box with items needed to complete requirements at the beginning of the semester.

It is the student’s responsibility to register for graduation before the deadline. Students, not course
faculty, are responsible for applying for graduation.

CLINICAL SKILLS
The following are clinical skills that students develop throughout the clinical courses:

• Conduct effective interviews of patients

• Perform accurate and complete history and physical examinations
• Interpret pertinent history and physical exam findings
• Document accurate history and physical exam findings
• Determine differential diagnoses from patient data
• Order appropriate diagnostic tests
• Develop relevant treatment plans
• Prescribe appropriate medications
• Analyze changes in the patient's condition
• Educate patients and family
• Make appropriate referrals

METHODS OF INSTRUCTION:
• Precepted Clinical Time at Approved Clinical Site
• Seminar
• Problem-based learning
• Review of Current Evidence Based Literature
• Review of Current Evidence Based Guidelines
• Clinical notes
• Simulation Lab
• Standardized Examination
• Board practice tests

EVALUATION:
Students must fulfill all assignments and requirements listed in the course syllabus and Course
Canvas Site by 4/30/2021 before 5 pm in order to pass the course.

Completion of required number of precepted clinical hours listed on their approved degree plan
and equivalent to the number of credits registered evaluating patients at approved clinical site(s)
and verified by preceptor by 4/30/2021 before 5 pm P/F

IMPORTANT: 6752 is a variable credit hour course (1-6). It is the student’s responsibility to
5

Revision Date: 12/30/2020
Review Date: 1/7/2019

ensure the student is registered for the correct number of credit hours to pass the course. Students
who register for hours not approved on the degree plan will have graduation delayed. Students who
are unsure about the number of credit hours for registration must contact the academic advisor as
soon as possible to verify accuracy of registration. Students who do not obtain the mandatory hours
for required specialties or preceptor requirements will not pass 6752 and/or have graduation
delayed.

As a reminder, one (1) semester hour equals 60 clinical hours. Students must meet the total clinical
hour requirement to pass 6752 and to graduate on time. Only the number of credit hours the student
is registered for in myUTH is what counts (for example, a student who registered for 1 credit hour
and completes 3 credit hours will only receive 1 credit hour credit per registration). Under or over
registration puts the student at risk for course failure and/or delayed graduation time. Please consult
your academic advisor for questions about the degree plan and/or hours needed for registration.
Refer to the academic and semester calendar for add/drop and withdrawal date deadlines.

Completion and submission of required clinical paperwork per syllabus requirements by 4/30/2021

Knowing falsification of any documents related to clinical experience will result in course
failure and dismissal from program due to academic dishonesty.

N6752 is Pass/Fail. The student must pass all of the evaluation measures in order to pass the
course. In the event that a preceptor fails a student for clinical performance, faculty reserves the
right to offer the student a second evaluation of performance. At faculty discretion, students
may be asked to complete more than the minimum number of required clinical hours to meet
course objectives. Only required number of hours will be counted for the course and program.

TOTAL PROGRAM HOURS

FNP Program Coordinator must approve all clinical rotations and experience prior to any
student entering the clinical setting. FNP Program Coordinator reserves the right to assess
appropriateness of clinical experiences in the form of approval or denial of rotation. Students
cannot modify hours without documented extenuating circumstance preventing the successful
completion of the rotation. Approval for each rotation must be marked as “Complete” with
approved comment in the Clinical Approval Site in Canvas.
Students must complete a minimum of 660 clinical hours (600 clinical hours for those
beginning the program prior to Fall 2017) to graduate from the FNP program which are rated
“Pass” by the clinical faculty.

The total required clinical hours for a Post-Master’s (PM) NP student who is changing specialty
(i.e.ANP or WHNP to FNP) is 540 hours (or as otherwise indicated on the degree plan) which
must receive a grade of “Pass” by the clinical faculty.

INCOMPLETE
All clinical hours, assignments, and paperwork must be completed by the date listed in the course
syllabus schedule (last page). The student must prioritize the completion of clinical hours within
the semester time frame (per learning contract date range). The inability to complete clinical hours
within the semester date range per the syllabus is not an acceptable reason for an incomplete
grade. Students who do not complete clinical hours within required time frame will receive a grade
of a “fail”. An “incomplete” may only be considered for inability to finish clinical hours due to
reasons outside the student’s control and with proof of an extenuating circumstance.
All evaluation documents (Typhon logs, preceptor and student evaluations to online course)
MUST be completed and uploaded to online course appropriate drop boxes by the due date listed
in the syllabus schedule or a grade of fail will be applied. A student who fails a didactic course
with a corresponding clinical/lab course MUST repeat the didactic course and corresponding
clinical course regardless of the grade received previously. Likewise, a student who fails a clinical
course with a corresponding didactic course MUST repeat the clinical course. The student must
also repeat the corresponding didactic course regardless of the grade received previously (Master’s
Program Policy G5670). All requirements, including tests and assignments, must be repeated in
the didactic course and all requirements, including the clinical hours, must be repeated in the
clinical course.
PREPARATION FOR CLINICAL

1. Approval for all clinical rotations in the FNP Clinical Approval course for the semester

2. Student is responsible for verifying that preceptor is listed in Typhon. Legal Affiliation
agreements are with sites, not preceptors.

3. The student must determine and then discuss clinical objectives and the best time for the
student’s precepted clinical experience with the preceptor prior to starting
the experience.

a. Choice of time will depend on i) availability of experiences, ii) convenience to the

preceptor, and iii) lastly the student’s personal schedule.

b. Students are required to provide preceptor with a copy of Preceptor Guide and all
clinical forms to set expectation for student learning.

4. Once the student meets with the preceptor and determines dates and times for practice,
student scans the completed and signed Learning & Clinical Hours Contract form as
a PDF file and submit the PDF file to the course online via the course drop box
located in Modules.

a. Submit the “Learning & Clinical Hours Contract” PDF file to the drop box
WITHIN 7 CONSECUTIVE DAYS OF THE FIRST DAY OF CLINICAL
DAY. If the Learning Contract has not been submitted before the end of the 7th
day of the first clinical day, the student will not be able to count ANY of the
clinical hours, regardless of late submission and the student can only count hours

going forward.

CLINICAL HOURS

1. Students must complete a minimum of the required hours listed on the approved degree
plan and registered credits of precepted clinical hours. Only the time added to the case
log will add up to the registered hours per degree plan of contact time. Time spent with
patients depend on the level of patient complexity. Do not separate time in Typhon for
time spent with patient and time spent with preceptor.
Low complexity (30 minutes can be counted)
Medium complexity (45 minutes can be counted)
High complexity patient (60 minutes can be counted)

2. Clinical hours do not include:

a. Travel time to and from site; lunch break time at site, other non-patient contact
related activities
b. Mandatory course simulation time is not included on clinical learning contracts or
clinical evaluations. Simulation is graded in the course and not at the clinical site.
3. Students may not use time spent at educational conferences towards clinical hours.
4. Clinical hours are to be evenly distributed throughout the semester. The student must
discuss any deviations in that schedule with course faculty before clinical hours are
confirmed with preceptors. Faculty decisions on distribution of clinical hours are final.
5. There are some clinical sites where the student needs to complete site specific
requirements. In such cases those hours are not included in the student’s precepted
clinical hours as they are not time spent with patients. Only hours completed with the
preceptor for the purpose of seeing patients are included in the stated clinical hours for
that site.
6. Clinical hours are not allowed outside the semester timeframe. Please do not date
Typhon time or case logs outside of the first day to last day of classes, as clinical hours
dated outside of the semester do not count.

STUDENT RESPONSIBILITIES

Students are expected to take responsibility for their own learning. Successful completion of this
course requires participation in Canvas course and Typhon in order to submit required
assignments and clinical rotations with preceptors to attain the required amount of clinical hours.
Students are required to monitor their progress throughout the semester and ask for help when
necessary.

1. Call the clinical site to schedule dates and times for clinical. If the student is unable to attend
clinic at the prearranged date/time it is his/her responsibility to contact the clinical site to make
other arrangements. Students must notify course lead for missing clinical due to calling in sick
or rescheduling two or more clinical days.

2. Dress: Students must comply with the dress code regulations in the clinical attire photo.
Each student must also have his/her own stethoscope, watch and other tools needed for the
individual site. Refer to the COVID-19 module for PPE requirements.
9
Approval Date: October 2014
Revision Date: 12/30/2020
Review Date: 1/7/2019

3. While in the clinical setting, the student is not an observer or shadowing: the student will (1)
complete comprehensive and/or interval health histories on selected patients, (2) perform head to
toe physical exams, (3) be able to identify relevant lab/special tests, (4) formulate a list of
differential diagnoses based on the health history and physical findings, (5) identify a plan of
care, and (6) chart in patients’ medical records utilizing the SOAP format (or a format the
clinic/practice has adopted).

4. Approval for clinical clearance with the site is through the FNP Clinical Approval Canvas
site. Students may not begin any clinical rotation without clearance through the FNP Clinical
Approval Canvas site. Students who complete hours at any unapproved site will not be able to
count any hours and will not meet course requirements.

OFF SITE Please communicate with lead faculty before having to go off site. On rare occasions, if the student and preceptor have to go off
site (i.e. to make hospital rounds), the student is required to drive his/her own vehicle and is not allowed to be in the same vehicle driven by the
preceptor due to insurance requirements.
Spring 2021 Semester: Due to the spread of COVID-19, students will not be permitted to go off
site with the preceptor (I.e. students in OBgyn rotation cannot go to a separate site to observe
births). The student will only be permitted to attend clinical rotations and participate in clinical at
the approved (physical address listed) in the Clinical Approval Course.
FINAL COURSE GRADE
Course lead faculty is solely responsible for assigning a pass/fail final course grade. The clinical
grade will be based on student evaluation by the lead faculty, input from preceptor, and site visit
faculty. In congruence with the National Task Force Criteria for evaluating NP student
competencies, the course lead faculty makes the determination if the student is passing the
clinical components and not the student’s preceptor. Faculty can fail a student in an FNP clinical
course if they are determined to be practicing unsafely, unethically, or exhibiting unprofessional
behavior at any time during the semester.

PASSING SITE VISIT

Students must have a passing site visit by a clinical faculty member during the semester to meet
the course requirements. It is the student’s responsibility to assure that the site visit is scheduled
with the clinical site visit faculty. Although site visits are usually arranged ahead of time, faculty
has the right to arrive/visit the clinical site unannounced. The student is to email the course
leader and the clinical faculty member (all in one email), if there is any difficulty arranging the
site visit. Students who do not pass at least one site visit with faculty will not pass the course.
PROFESSIONALISM

Students are always expected to conduct themselves professionally in the academic and
clinical setting. Professional conduct includes communication, verbal and non-verbal.
Students must achieve a score a 100 percent on the Clinical Professionalism module
assignment to pass the course– see rubric in Canvas. Students who score less than a 100 on the
Professionalism Assignment will have a score of 0 reported for the final course grade and will
not pass 6752.

Office hours for the faculty members are by appointment only. Contact information is listed in
the syllabus. For emails sent Monday through Friday (during normal business hours), expect
response within 24-48 hours. For emails sent on weekends and holidays, expect response within
48-72 hours on a business day.

COURSE COMMUNICATION

Students are expected to check their UT email and Course Announcements daily during the
week for communications regarding course updates. The course announcements section in
Canvas will be used to call your attention to important course related information or changes.
Please check Announcements daily. Any necessary changes to the course schedule will be
posted under Course Announcements for which students will be responsible for knowing.
UTH Outlook email is the preferred method of communication.
All questions about clinical site approvals, clinical/course requirements, and registration (number
of hours registered for the course) must be directed to Dr. Kobeissi for guidance and/or clearance
of clinical hours and/or clinical sites. Site Visit faculty cannot provide students with approval or
modification of clinical hours. Any changes to the course requirements or clinical site approval
that are not approved by Dr. Kobeissi and are not consistent with program or course requirements
will result in an unsuccessful course attempt.
Participation in a cooperative learning environment is a part of this course learning
expectation. A variety of class activities are planned. Some of these activities are based on
cooperative learning techniques. Students should be prepared to contribute to building an
engaging learning community in various settings. Students are expected to contribute to the
discussion and inquiry, be responsive, be respectful, and limit themselves to criticism that is
constructive (advice that is useful and intended to improve or that offers possible solutions).
the course and clinical setting. Clinical Conferences will be scheduled at faculty’s discretion and are mandatory. Clinical faculty reserve the right to ask for add

CLINICAL PRESENCE AND ABSENSE

Students must notify course lead in the event the student has more than two (2) clinical absences at
a clinical site in one semester.
1. The student is expected to complete the agreed upon clinical schedule as negotiated with
the clinical preceptor:
a. Failure to meet the clinical schedule due to unexcused/unplanned absence(s) will
result in withdrawal from the clinical site and failure to progress in the program.

2. If the student must miss a clinical day, they must:

b. Notify the preceptor prior to the scheduled date. Students who do not call and do
not show to scheduled clinical day(s) will be held to the professional assignment in the
course.
c. Notify (by email) to lead faculty, site visit faculty of the student’s absence that day.
Students who report a clinical absence due to illness will require a note from a
healthcare provider in order to be cleared to return to the clinical setting.

Students are expected to accurately report clinical attendance and hours in Typhon and on
11

Review Date: 1/7/2019

evaluations. Students cannot count any missed clinical days towards clinical hours. Students are
required to update the Typhon schedule within 24 hours of a change to clinical attendance. Students
who falsify clinical attendance and/or Typhon records will not be able to count any clinical hours
for the semester, will be removed from the clinical setting and fail the course. The student will be
held to the professionalism grade and academic honesty policy.

SITES WITH SPECIAL REQUIREMENTS

Please refer to Canvas site (Clinical Resource Center) to verify if your site has special
requirements. The student is responsible for ensuring clinical special requirements are met prior
to day 1. Do not begin clinical unless you have received onboarding clearance to begin the
rotation. Students who are not compliant with special requirements will not be able to count any
hours towards the clinical hour total. The student must complete an Onboarding Application
Request if the site has special requirements so that Mr. Baker can process paperwork before
beginning a rotation. Students who are not compliant with special requirements will not be able
to count any hours towards the clinical hour total.

E*VALUE

The School of Nursing is using a system called EValue to track student immunizations and
certifications. Students will be responsible for uploading digital copies of their immunization
and certification documentation. This information is required by clinical sites before you can
start your clinical. Please see attached detailed instructions for how to set up and access E*Value
to enter your information. This information may be found online at https://nursing.uth.edu/evalue

Students can email Evalue questions to: MSNPMEvalue@uth.tmc.edu

CLINICAL EVALUATION FORMS & TYPHON DOCUMENTATION

The student initiates and maintains an electronic clinical log throughout each precepted clinical
experience to document the types of patients seen and clinical skills and procedures that were
accomplished. The records must be de-identified. Students are to post logs at least weekly. If a
student falls more than one week (24 total Typhon hours) behind in posting clinical logs, the
student will be removed from the clinical setting until such time as the logs are brought current.

CLINICAL FORMS

Clinical forms must be TYPED and submitted in PDF format and document titled the following
way: “LastName_First Name_FormTitle.pdf”
Online course drop boxes
12
Approval Date: October 2014
Revision Date: 12/30/2020
Review Date: 1/7/2019

◦Online course drop boxes are used in this course to submit scanned documents
outlined below
◦All documents must be saved in PDF format. No other format is accepted including
photographs saved as PDFs.
◦Only submit one document per drop box provided. If an entire document consists of two
pages, please upload both pages into one drop box, e.g Learning Contract is a two
page document and has to be submitted in its entirety into a “Learning Contract” drop
box – both pages
◦A scanner is available on the 3rd floor in the computer labs (room 310).

PAPERWORK SUBMISSION
It is the student’s responsibility to ensure documentation is per the syllabus. Clinical paperwork
that requires revision must be recompleted. Modification of an already signed document (I.e.
preceptor signature listed) will not be accepted. Revisions require a student complete another
document and have it resigned.
Submissions sent/received by email are not acceptable. All paperwork must be submitted through
Canvas dropbox - do not attach course documents in the assignment comments section unless
otherwise requested by lead faculty.

TYPHON AUDIT
Typhon audit deadlines are firm. Students who do not submit an audit on time must stop rotations
until Typhon audit is submitted. Typhon audits must reflect that students are following the syllabus
to continue in clinical rotations.

Audits that are not passing will require revision before the deadline listed in the comments section
to continue in the clinical setting.

TYPHON

Student is responsible for verifying that site is in Typhon: correct practice name, correct address,
LEGAL affiliation agreement with UTHealth is current. No clinical hours will be used if
completed at a site with no legal affiliation agreement. The student maintains clinical logs in
Typhon throughout the clinical experience to document their schedule, types of patients, and
skills. Logs must be completed weekly or student can be removed from clinical. Students are
expected to use the “Student Video Tutorial” located within Typhon (login to Typhon, and
search for link to tutorial) to learn how to insert data. Link to Tutorial is also found in Student
Clinical Guide. Each menu item on the left will link you to a different area of the video tutorial
as a segment.

Dating of case logs: When charting case logs, use the SUNDAY preceding the day in clinic on the
case logs for privacy reasons. For example, if a student is in clinical on Tuesday, Thursday, and
Friday, when documenting all 3 clinical days, the student will use the Sunday of that week as the
date of the case log encounter. The case logs are dated on Sunday, whereas the student’s full
schedule will be under the Typhon schedule. Do NOT date Typhon time logs (shift and patient
time) or case logs outside of the dates of the first day of class to last day of class per the Cizik
School of Nursing semester calendar. Any Typhon record dated outside of the scope of the
13

Review Date: 1/7/2019

semester will not count (if you begin clinicals on week 1, you can use the Monday date for
that week).
CONFIDENTIALITY:

An important part of nursing ethics is maintaining the client’s confidentiality. Therefore, written
work submitted to the instructor must never contain the client’s full name or identifiers. Client’s
problems must not be discussed with family or friends. If the Department ascertains that a
client’s confidentiality has been violated, the student violating the confidence will be subjected to
disciplinary action, including failure of clinical course.
Students are not permitted to post clinical site, preceptor, or patient information on any social
media (to include photos, reviews, feedback, etc.). Students who fail to maintain patient and/or
clinical site privacy will fail the course. Students should contact course lead faculty with any
concerns as soon as an issue is identified.
Keep a copy of Typhon logs/clinical paperwork for personal records.

ATTENDANCE

Class Attendance: Students are required to attend on site experiences, and to arrive at class
experiences on time and on schedule.

SIMULATION/STANDARDIZED PATIENT/ INTERPROFESSIONAL EDUCATION

EXPERIENCES

Mandatory Simulation/SP/IPE Labs will be held at UTHealth facilities. More information

provided in Canvas course. Students are responsible for signing up for SIM/IPE when

date/time/group.

SIM Dates/times Firm. Arrive 15 minutes prior to start time to sign in. On time to SIM
means being on time to sign-in timings and not start time. The faculty and/or the medical
school reserves the right to switch schedules and student rosters. Students who are late to the
activity after the start time listed in the schedule will not be able to participate.

No Makeup SIM. There are no make ups for SIM/LAB activities. A failing grade will be assigned in
the course for missed lab assignments or SIM activities. Students may find a classmate to switch with
and notify course lead in case there are scheduling conflicts. Both students must agree and be copied on
the email to lead faculty for permission to switch SIM lab date. IPE dates and times are firm. Arrive 15
minutes prior to start time to sign in. On time to SIM means being on time to sign-in timings and not
start time. Students are not allowed cell phone usage during SIM/IPE. This activity should be treated as
an exam.

One passing SIM day is required to fulfill the requirements to pass 6752. Students who receive an
excused absence (see criteria for excused absence) for SIM attendance may qualify for an Incomplete in
the course. The student must complete the SIM requirement in the immediate next semester to fulfill
the SIM requirement for FNP Preceptorship.

TESTING

Examinations are administered on specific dates noted in the syllabus must be attended. Students are
required to obtain the required password to access the testing platform. Course faculty do not have access
to student username or passwords. If a student is unable to sign-in to take the APEA or HESI, the student
will have a score of a zero reported for the activity. Students are required to view the tutorials for software
before the examinations.

Course examinations will be based FNP content (FNP 1 and FNP 2 materials). There will be NO make-
up exams. In the event of a missed examination due to unexcused absence, the grade of “F” will be
awarded. Students who receive an excused absence (see criteria for excused absence) for mandatory
examinations may qualify for an Incomplete in the course. The student must complete the exam
requirement in the immediate next semester to fulfill the exam requirement for FNP Preceptorship.
Students may only be excused from one examination to qualify for an incomplete, I.e. cannot miss
APEA and HESI in the same semester. Students who must miss more than 1 mandatory examination
must discuss course progression with course lead faculty and their academic advisor.

HESI and APEA EXAMS

Exams Sign-up. Students must sign-up to take the HESI exam at the beginning of the semester and by
the end of the first week of classes to meet course requirements. In Canvas, go to “People”, select
“Group” and sign up for an exam room or to test virtually. Students will not be able to switch testing
platform after sign-up for HESI. Students are responsible for reviewing exam date and start time, as
each exam start time is different. Students who arrive late to test (15 minutes after posted scheduled
date/time in syllabus schedule) will not be able to sit for the examination and will receive a score of a
zero (0).

• APEA

Students are required to register for APEA QBanks, pre-predictor exam, and complete APEA tests

following instructions posted in Canvas. Students cannot modify registration instructions posted in
any way for any reasons (for example, if students are instructions are to select X number of months,
registering for a different number of months will make registrations not valid). The student must
upload APEA receipt into Canvas before the due date to meet the course requirement. The pre-exam
will indicate base knowledge in the expected FNP primary care competencies for a safe practice.
APEA Pre-Predictor Exam is on the scheduled dates per course calendar. The test is available online
through the assigned APEA Webpage. APEA will email student the instructions for taking the exam.

• HESI

Students must sign up to take HESI on campus or ProctorU before the deadline in the syllabus. Students
who opt to take the HESI on campus must sign up before the date listed on the syllabus. Missing the
deadline results in the student being required to take HESI on ProctorU. Students are unable to switch
testing deadline after the sign-up deadline.

Absences. In the event of an expected/unexpected absence, the student will notify the instructor as soon
as possible, with at least 24 hours before the start of the exam to receive an excused absence. Failure
to notify faculty promptly will result in an unexcused absence, and the student will earn the score of
zero for the exam.

Excused absence:
• Death of an immediate family member (first degree relative)
• Death of a spouse
• Hospitalized child with a MD/PA/NP written and signed note
• Illness with a MD/PA/NP written and signed note
• Motor vehicle accident while on the way to school (police report required)

Not considered an excused absence:

• Vacation
• Unprepared for exam (not ready to take it)
• Computer issues (ie. Using a Macbook/Apple and not a Windows)
• Internet connectivity issues

No other excuses will be approved. If a student cannot take the HESI, the student will not be able to
pass the course until the HESI exam is successfully taken.

There are no makeup exams in 6752.

Exam and lateness. If a student arrives late to the examination test room or SIM activity on the day of
a scheduled exam or attendance, the student will not be able to complete the required activity and will
be given a score of a zero (0) for the assignment/test. Any student that arrives 15 minutes or later than
the scheduled time will not be allowed to sit for the exam or perform SIM activity.

ACADEMIC SUCCESS

The course faculty offer students the opportunity to be successful in academic studies. It is
possible that personal events and learning styles can interfere with one’s ability to learn and
therefore maintain passing grades in a course. Throughout the semester, the student is responsible
to track progression of his/her grades. If the student has difficulty with assignments or falls below
a passing grade, the student has the responsibility to contact faculty and determine a plan to
maintain passing grades. In the event of any academic difficulty, resources are available in the
School of Nursing to help the student achieve. The student should consider the following:

1. Contact course faculty to discuss academic progress.

2. Contact the student’s academic advisor.
3. Work with the UT Health SON’s Graduation Coach on test taking, writing skills and/or
tutoring assistance.
4. Contact the free student assistance and counseling services at
http://publicaffairs.uth.tmc.edu/worklife/index.html. Click on the “contact us” button or the
student can call 713.500.3327 or e-mail: uteapmgmt@uth.tmc.edu.

ADD DROP AND WITHDRAWAL DATES

The student is responsible for knowledge of add/drop and withdrawal dates during the semester.
The student is responsible for communicating with the assigned academic advisor to timely initiate
a successful course withdrawal. Students who withdraw from N6752 after the drop date will have
the current N6752 grade attached to withdrawal, which may impact academic standing and
GPA. Please note that initiating a withdrawal is not complete until the student ensures that
academic advisor and lead faculty have signed the drop request. The student is responsible for
following up with the student affairs office to ensure successful withdrawal, not the course faculty.

ACADEMIC HONESTY

Students are expected to be above reproach in all scholastic activities. Students who engage in
academic dishonesty are subject to disciplinary penalties, including failure in the course and
dismissal from the university. “Scholastic dishonesty includes, but is not limited to cheating,
plagiarism, collusion, the submission for credit of any work or materials that are attributable in
whole or part to another person, taking an examination for another person, any act designed to
give unfair advantage to a student or the attempt to commit such acts.” (The University of Texas

• Any student who commits an act of academic dishonesty is subject to discipline. Penalties
may include but are not limited to failure on the assignment, failure in the course, suspension
from the program, or expulsion from the school. Note that there is zero tolerance for
academic dishonesty in N6752.
• Reconstruction of the course exams either individually or within a group OR
participation in any activity which gives students an unfair advantage over other students
is prohibited. Students found guilty will be subject to disciplinary action.

STATEMENT ON PLAGIARISM

Plagiarism is using another person’s work or ideas without giving credit. Plagiarism is dishonest
because it misrepresents the work of another as your own. This includes both copying the work
verbatim or re-phrasing the ideas of another without properly acknowledging the source. When
preparing work for course requirements, such as papers, care plans, take home examinations,
etc., students must be careful to differentiate between their ideas and language and information
derived from other sources. Sources include but are not limited to published and unpublished
materials, the Internet and information and opinions gained directly from other people including
faculty or other students. Whenever ideas or facts are obtained from a student’s reading and
research, the sources must be properly cited according to standards set by the School of Nursing
and contained in the current edition of Publication Manual of the American Psychological
Association.

Plagiarism is considered a violation of academic integrity and professional honesty. Students

who are found to have plagiarized may be subject to disciplinary action. Penalties may include
but are not limited to failure on the assignment, failure in the course, suspension from the
program or expulsion from the school. Collaboration in the completion of written assignments is
prohibited unless explicitly permitted by the instructor. Students must acknowledge any
collaboration and its extent in all submitted course work.

Publication Manual of the American Psychological Association. 6/20/03

DISABILITY ACCOMMODATION

An attempt will be made to provide reasonable academic accommodations to students that

request them in accordance with 504/ADA guidelines. See the UTHealth Disability
Accommodation policy for details. Please provide disability accommodation documentation to
the course section faculty the first week of the semester in order to guarantee separate testing
room facilities.

zero. Students who do not submit an assignment due to being removed from a course as a result of
financial issues will not be offered a makeup assignment.

COVID-19 Requirements

FNP Students will be required to fully complete the COVID-19 module located in the Canvas
Clinical course along with the University COVID-19 survey to receive clearance prior to the first
day of clinical rotations. Students who are not in compliance with the COVID-19 module,
attestations, or daily clinical log will be removed from the clinical setting.

Students who do not submit the daily clinical log will not be able to count clinical hours until the
student is in compliance with self-monitoring of temperature and signs and symptoms of COVID-
19.

UTHealth graduate clinical students will be provided with additional resources addressing the
proper donning and doffing of PPE https://www.cdc.gov/coronavirus/2019-ncov/hcp/using-
ppe.html

Graduate students should self-monitor for signs/symptoms of COVID-19. Students will check their
temperatures every morning prior to clinical and evening and remain alert for other COVID-19
symptoms (fever, cough, shortness of breath, sore throat, myalgia, malaise). They should self-report
any of these symptoms to their clinical instructor and seek further instruction and evaluation at the
UTHealth Student Health Services.

Refer to the daily clinical quiz in the COVID-19 module in the Canvas Course.

Assuring adequate PPE:

UTHealth clinical students should adhere to CDC guidelines of proper PPE to protect against
COVID-19. Students are to follow clinical site policy for PPE while on site. At minimum, face
masks should be worn during clinical experiences, as well as additional PPE depending on
setting/situation requirements. If a clinical site is unable to provide protective face masks, the
student is responsible for providing his/her own. Additional PPE requirements will be provided by
the clinical site. Hand sanitizer and soap/water should be utilized prior to and after each patient, as
well as any time hands could potentially be unclean. Students will be instructed to remove
themselves from any situation in which they believe they are not adequately protected and to notify
course faculty.

No contact with COVID-19 patients:

UTHealth graduate clinical students are not to participate in the care of any patients diagnosed with
COVID-19 (SARS CoV 2).

Students who encounter COVID-19 positive persons at any time and without wearing of proper

PPE will be required to self-quarantine and contact course faculty for instruction. The student will
be directed to UTHealth Student Health Services for further evaluation.

Access to testing:

CSON student evaluation and testing will be provided by UTHealth Student Health Services.
Additional community-based testing is also available to students if needed.

Removal from Clinical:

Students who feel unsafe in any clinical setting should remove themselves from an unsafe situation
and contact the FNP Track Coordinators immediately for assistance.

TEMPERATURE LOG AND COUNTING CLINICAL HOURS

FNP students are required to monitor and report their temperature within 1 hour before the clinical
start time and monitor temperature again in the event of symptoms consistent with COVID-19.
Students must take and report their temperature in this Canvas quiz no earlier than one (1) hour
prior to clinical in order to count clinical hours. Quizzes submitted after the clinical start time will
result in the student not being able to count clinical hours for the day.
CERTIFICATION EXAM PROCEDURES
For questions about the certification exam, please review the wrapping up module in the last
module of the 6752 course. Students are encouraged to review the information for initial
certification on the AANP and ANCC websites. Become familiar with testing procedures for both
organizations. Faculty cannot select exam type for students.
It is important that you complete your certification application correctly to avoid correction delays.
ANCC Validation of Education forms and AANP program completion letters cannot be sent to “sit
early” for the boards until all course requirements are fully complete (see deadline when forms are
accepted in the course schedule – last page syllabus). AANP does not require a letter to sit for the
boards early within the 30 day to degree conferred date – if you are unable to sit within the 30 day
window, check your application for errors in reporting completion dates; corrections for student
errors that are required by faculty cannot be processed until all course requirements are completed
and reviewed.
Students testing with ANCC and require the ANCC Validation Form must fill out the top portion
of the form in its entirety. Use the dropbox in the Wrapping Up Module in the 6752 course and
allow 3 full business days to complete. Incomplete forms will be returned to the student, please be
sure you complete the form fully and accurately.
