Nurs 492- Community Health Nursing

Purpose of Assignment

The purpose of this assignment is to allow students the opportunity to gain entry and assess a
population within their community.  This is an independent assignment.  Students will select a
community-based coalition, school, civic organization, occupational health setting or other
practice site for this clinical experience and work with this community to identify and prioritize a
community diagnosis and develop an implementation plan to address it.  The purpose of this
assignment is to involve students in the comprehensive community needs assessment. Students
will develop/utilize tools to identify perceived factors (both negative and positive) that impinge
on the population’s health. This information will then be used to develop and implement
strategies for health promotion. 

Student Approach to Assignment

I researched civic organizations in my area and found the Community Resource Center of
Nashville Tennessee. I reached out to head of this organization about the purpose of the
organization and the needs of my community. I used this information to identify a community
diagnosis and developed a plan to address the diagnosis.

Reason for Inclusion

This assignment was included because it taught me how to find resources needed within my
community based off the people’s needs of the community.

Leadership

 Initiates community partnerships to establish health promotion goals and implements


strategies to meet those goals.
I met this objective by forming a community partnership with the Community Resource Center
of Nashville Tennessee to find out the needs of the community. The needs of the community
include ease of access to mental health treatment, adequate food sources, and affordable housing.
I used Maslow’s hierarchy of needs to develop nursing diagnoses related to the needs of the
community. The strategies that were implemented were people living in poverty will be able to
explain the amount of water needed to maintain hydration. They will also be able to seek out
resources for healthcare, as well as trade schools and job fairs. People living in poverty will be
able to locate other resources besides the Community Resource Center of Nashville Tennessee
for help with affordable housing.

After I assessed the needs of the community, I used Maslow’s hierarchy of needs to develop
nursing diagnoses related to the needs of the community. I then organized the nursing diagnoses
based on the most important first. I created smart objectives related to the nursing diagnoses and
incorporated them into a teaching session. I evaluated the learners’ attainment of information
through questioning. I asked them to name places within the community where they could get
food and other resources. Some examples they gave me included the Community Resource
Center of Nashville Tennessee, the food bank, and churches.

I met this objective because in this assignment I had to connect with a community aggregate to
find the needs of the community. I used Maslow’s hierarchy of needs as a guide to developing
my nursing diagnoses related to the needs of the community. I then used the nursing diagnoses
to develop smart goals which would help meet the needs of the community. One nursing
diagnoses that I developed was people living in poverty are at risk electrolyte imbalance due to
inadequate sources of nutrition. After the lesson I asked them to give an example of a complete
meal to reduce their risk of electrolyte imbalance. I also asked them to tell me places of where
they can go for resources if needed.
