JOINT-AFFIDAVIT

LUALHATI ST., BRGY. 31, ZONE 2, TONDO MANILA respectively and with cellular
phone numbers 09178399010 and ________________, after having been duly
sworn in accordance with law, hereby depose and state that:

1. I, JOSE VALSOTE BITHAO, am a Certified Public Account in public practice with

PRC License No. 53300 issued on _______ and with the PRC – BOA Certificate
Number 099 issued on AUGUST 08, 2018.

2. That in the course of my practice as an independent auditor, I was engaged by

HARMONICS ELECTRICAL SERVICES to perform an audit for its Financial
Statements for the period ending _____________;

3. I, EVELYN CASTAÑEDA MENDOZA, am the Authorized Managing Officer of

HARMONICS ELECTRICAL SERVICES, a construction firm with office address at
2102-B WEST TOWER, PSEC, EXCHANGE ROAD, ORTIGAS, PASIG CITY.

4. That HARMONICS ELECTRICAL SERVICES is a holder of Contractor's License No.

38500, originally issued on JANUARY 24, 2013.

5. That we are jointly executing this affidavit in support of HARMONICS

ELECTRICAL SERVICES’s on-line renewal of PCAB Accreditation;

6. That we attest to the existence and due execution of Financial Statements (with
Notes to Financial Statements) for the taxable period ending __________ of
HARMONICS ELECTRICAL SERVICES, dated/approved on ____________ by
the board/management.

7. I, JOSE VALSOTE BITHAO, have audited the said Financial Statements and issued
an Unqualified Opinion that the assertions contained therein are fair and
 reasonable and are in accordance with the prevailing acceptable standards of
accounting.

8. That I, EVELYN CASTAÑEDA MENDOZA, states that the Audited Financial

Statements referred to above by the Auditor was made the basis in the submission
of the Annual Income Tax Return and that the same will be/was submitted to the
Bureau of Internal Revenue without any necessary corrections and/or amendments.

9. Thus, we hereby jointly swear that the same Financial Statements with Notes and
Independent Auditor’s Opinion was made the basis for the on-line entries for the
renewal of accreditation with the Philippine Contractors Association Board. Further
that the on-line application was performed under our joint supervision, control and
responsibility;

10. That we have examined and validated every entry and information supplied in
the on-line application with the above Audited Financial Statements and we hereby
confirm the same to be true and correct and should the Board require the
submission of the Audited Financial Statements, a copy thereof consisting of
______ ( ) pages will be submitted.

11. We are fully aware that:

a. Any discovered misrepresentation of information encoded or attached

as a document and/or manifestations of fraud on the application
documents submitted by my firm applicant or its Authorized
Representative/Agent/Liaison Officer shall be subjected to
investigation.

b. Unconfirmed information/documents submitted to support my firm's

qualifications shall be excluded for categorization/classification
purpose; and

c. The evaluation of the firm’s qualifications shall be solely based on the

documents submitted at the time the application was filed/accepted by
PCAB.

12.WE further certify that we prepared this document that we fully understood every
word of it and its meaning, and have affixed my signature hereunder voluntarily and
 freely with the full and complete knowledge of the meaning and intent of this
document as well as of my rights under existing laws.
