AFFIDAVIT OF DISCREPANCY AND EXPLANATION

I, JOHN MARK SAMPIANO VITO , of legal age, married, Filipino and a resident of
Bacalan,Sebaste, Antique, Philippines under-oath depose and say that:

That I am a registered owner of the account at Philippine Overseas Employment

Administration (POEA) with ERegistration Number: 2018110300033.

That my true and correct date of birth is on November 22, 1987.

That I discovered my date of birth was erroneously recorded as Nov. 21, 1987 as
recorded in my Worker’s Information Sheet ERegistration Number: 2018110300033 instead of
my true and correct date of birth on Nov. 22, 1987.

That the said discrepancy of name in my Worker’s Information Sheet was advertently or
typographical error as Nov. 21, 1987 instead of Nov. 22,1987.

That I undertake that my date of birth on Nov. 21, 1987 and Nov. 22, 1987 refers to one
and the same only;

This error was committed through inadvertence, honest mistake or excusable negligence
but not for the commission of fraud;

That I execute this affidavit in good faith and attest to the truth of all narrated facts, for
whatever legal purpose it may duly serve.
