CHAPTER I. BACKGROUND

1.1 Situation Analysis


At the global level a global partnership initiative for Family Planning is
known as Family Planning 2020 (FP2020). FP2020 aims to support the rights of
each woman to be able to freely determine themselves, whether they want to have
children, when to have it, and how many children you would like to have. FP2020
works with Governments, civil society, multi-lateral organizations, private parties,
and research institutes and development partners to allow an additional of at least
120 million women (additional users) to use contraceptives in 2020 (Direktorat
Jenderal Bina Kesehatan Ibu dkk., 2014).
In Indonesia, Early childhood marriages are still an issue in some
districts / cities as well as Increased proportion of married groups at 16-18 years
of age. Based on data from 2015, there are 20 provinces with a higher prevalence
of child-aged marriages than national numerals of 22.82%. The marriage age has
been generally increased as can be seen in the increase of the wedding age from
17.1% in 1991 to 20.1% in 2012, the high proportion of early marriage and
pregnancy / childbirth levels at an early age (Badan Pusat Statistik, 2016).
Social phenomena about young marriage in Indonesia is one of the more
common phenomena in the country both in urban and rural areas. Child-age
marriages in rural areas are almost a third higher than in urban areas respectively
27.11 percent and 17.09 percent in 2015. The high number of child marriages
raises a variety of maternal health problems that are transferred under 18 years
have a 35% to 55% higher risk for giving birth to a baby. Babies born to mothers
less than 20 years are also at risk of premature birth, low birth weight (BBLBR),
and also congenital or defects that have occurred since the pregnancy process
(Badan Pusat Statistik, 2016; Oktavia dkk., 2018).
Based on the Projection of Indonesian Population 2010-2035, Indonesia's
population has increased from 238.5 million in 2010 to 265 million in 2018. This
increase in population can be seen through the number of births. Data from the
SDKI 1991 shows a total fertility rate of 3.0 children. This means that an average

woman in Indonesia gives birth to 3.0 children during a woman's reproductive


period. However, in the 2017 RPJMN survey, the use of family planning methods
is decreased, namely 59.7 percent (Gerintya, 2018)
The National Population and Family Planning Agency (BKKBN) of East
Java Province said that the number of active Family Planning (KB) participants in
East Java as of March 2016 reached 5,907,243 acceptors (Widya and Setiawan.
2016). In 2016, as many as 369,064 people from Jember became family planning
participants. While in 2014 there were 399,646 people and in 2015 there were
393,331 people. Based on the 2016 Regent Accountability Report, independent
family planning participants reach an average of 32-33 percent per year from all
active family planning participants (Wirawan, 2017).
Based on the results of the assessment conducted by group 3 conducted on
29 November 2019 in the family of Mr. Ariel in RT 001 RW 002 Sumbersari
District Jember. Found the following data is a couple who don't follow the Family
Planning (KB) program because the family still embraced the traditional mindset
that is assuming when it is late married then the family will feel embarrassed and
cause many marriages in the age Young, further thought that a child is the
expectation of a parent as a breadwinner, the idea that the child is regarded as a
source of family to help parents and also the assumption that the child is the pride
of parents. The three views many children have many advantages so that many
families neglect the government program to implement family planning. The
fourth is the natural nature of man who wants to defend his descendants so that
family planning programs are still often ignored.

1.2 Formulation Issues


Based on the problem analysis above, can be withdrawn a problem from the
family of readiness for enhanced family coping.

CHAPTER II. OBJECTIVES AND BENEFITS

2.1 Objectives
2.1.1 General purpose
This health education activity aims to help improve the health status of the
family, supporting the reduction of maternal mortality rate (AKI), infant mortality
rate (AKB).
2.1.2 Spesific Purpose
a. Mr. Ariel's family is able to know the importance of KB.
b. Mr. Ariel's family is able to know the kinds of contraceptives.
c. Families can mention the advantages and disadvantages of contraceptive tools.
d. The family can choose the contraceptives to be used.
e. Family knows the impact if not using KB.

2.2 Benefits
2.2.1 Families are able to choose the right contraceptives and avoid unwanted
pregnancies, gaining ease to obtain information and basic health care.
2.2.2 For cadres, posyandu officers and public figures such as improve the health
status of the family and reduce the risk of infection with cervical cancer. Can
realize the actualization of himself in helping the community solve health
problems.
2.2.3 For health care such as optimization of Puskesmas's function as a center for
health-oriented development, Primary Health Care center and Primary Public
Health Service center. Can be more specific to help people in solving health
problems according to local conditions. To bring close access to basic health
services to the community.

CHAPTER III. TROUBLESHOOTING FRAMEWORK

3.1 Basic Thoughts


The MDGs report indicated that Indonesia did not succeed in achieving the
MDG objectives to reduce maternal mortality (AKI). Even the last Eestimation
tends to indicate an increase in AKI level. Family planning is one of the most
important interventions for lowering AKI and contributing to lowering about one-
third of the maternal mortality rate. As discussed in the previous section,
unwanted pregnancy in Indonesia revolves around 20% in married women,
mainly because of the disuse of contraception or short-term methods of
contraception. The consequences of unwanted pregnancies such as abortion and
its complications are known. Therefore, reducing unwanted pregnancy through
quality KB services can contribute to improving maternal health (Direktorat
Jenderal Bina Kesehatan Ibu dkk., 2014).
Therefore the government proclaimed the family planning program, KB is
one way to suppress the birth rate, so as to reduce the percentage of maternal
mortality rate. The use of contraceptives is important to control the birth rate.
According to the United Nations, by regulating and controlling the number of
births, family economy and welfare can be stronger. Its cumulative benefits are
contributing to poverty reduction and positive impact on development. As for
some ways in conducting health promotion regarding the KB in the family of
young couples among others understanding KB, objectives and benefits of KB,
the impact of not using the KB, various kinds of contraceptives, advantages and
disadvantages of contraceptives.

CHAPTER IV. ACTIVITY IMPLEMENTATION PLAN

4.1 Realization of Problem Resolution


We will do it on November 30, 2019 at 11:00 at the house of Mr. Ariel with
counseling activities to the family of Mr. Ariel in increasing knowledge about the
importance of Family Planning. According to our group the problem solving plan
for Mr. Jufri's family is by providing education to Mr. and Mrs. Ariel about family
planning and showing examples. Our group will also show educational flip sheet
about family planning so that the family of Mr. Ariel can more understand.

4.2 Target Audience


The target audience is Mr. Ariel's family, especially for Mr. and Mrs. Ariel.

Family Planning Program


a. Definition family planning (KB) is an effort to regulate the birth of children,
the ideal distance and age of childbirth, regulating pregnancy, through promotion,
protection, and assistance in accordance with reproductive rights to create a
quality family (Dewan Perwakilan Rakyat dan Presiden Republik Indonesia,
2009).
b. The purpose of the KB program (Dewan Perwakilan Rakyat dan Presiden
Republik Indonesia, 2009). :
1) General purpose
Improving the welfare of mothers, children in order to realize NKKBS
(Normal small Family Happy Prosperous) which is the basis of the
realization of a prosperous society by controlling birth while ensuring
the control of population.
2) Special Purpose
Increase the population to use contraceptives, The number of infant
birth numbers decreases, Increased health of family planning by the
way of birth looting.
c. Benefits of the KB for married couples and children (Darmawan, 2018):
1) For married couples :
a) Lower pregnancy risk
Contraceptives function to prevent unwanted pregnancy.
Contraceptives also serve to lower the risk of childbirth too
young or too old. If a woman who is too old and yet menopause
has an intimate relationship without using a contraceptive
device, there is a possibility of pregnancy. Giving birth over the
age of 35 years will be risky in women and can lead to death.
b) Decrease the risk of cancer in women
Hormonal contraceptives used by women, such as injectable
types, pills, or IUD usually contain progesterone and estrogen.
This hormone can help women control pregnancy and lower the
risk of cancer in the reproductive system.The cancer that can be
solved by two hormones is ovarian cancer (ovary) and kaker or
uterine wall (endometrium). Hormonal KB programs can also
lower the risk of uterine growth in the uterus.
c) Does not interfere with child growth
If the child has not been one year already has a sister, child
growth will be interrupted. Normally the first and second child
distances between 3-5 years. If the child is not yet 2 years old
already has a sister, breast milk for children can not be full 2
years so the possibility of experiencing health problems. Parents
with two children will also have difficulty dividing time. Then
the bigger child will be less attentive, although the child still
needs the full attention of both parents.
d) Decreases the risk of pelvic inflammation
Hormones for KB can be beneficial to decrease pelvic
inflammation. The inflammation of the pelvis affects the area of
the uterus, ovaries, and other vaginal areas. The risk of pelvic
inflammation is decreased if women use the type of implant KB
program. Tubectomy also lowers the risk of pelvic disorders that
can harm women's lives.
e) Maintain mental health
Some women may experience a considerable depression after
childbirth. Depression is usually lost if it gets support from a
pair. In the event of a child's birth at a close distance, the risk of
depression is greater. Depression can also occur in fathers
because it is not prepared physically and mentally. These two
conditions can be eliminated by conducting the family planning
program. If you do a pregnancy arrangement, a married couple
can live healthier. Even the child can grow maximally and the
planning of pregnancy will run smoothly.
2) KB Benefits for Children:
a) Can know the growth of children and their health.
b) Get attention, maintenance and adequate food.
c) Future planning and good education.
d. Types of contraception (Setiawati dkk., 2017)
1) Birth Control Pills
Should be taken every 1 tablet on a regular basis so that the bias
prevents pregnancy, if the mother takes a pill, ovulation or release of
eggs by daily ovulation Ovation, of course, will not occur pregnancy.
The pill should not be used for women with a characteristic of more
than 35 years and smoking, body fat, suffering from diabetes, heart
disease, cancer, diabetes, moigran, abnormalities of liver function.
a) Advantage:
(1) Birth control pills are easily obtained, effective when used
appropriately and correctly.
(2) Use of birth control pills may reduce the risk of
developing uterine cancer.
b) Lack:
(1) Daily Drinks.
(2) There are side effects that may be experienced by some
users of birth control pills such as nausea, dizziness,
vomiting.
2) Injection of KB
KB injections should be used regularly in order to obtain the results as
expected. The time to get the reinjection should be in the client recall.
Regular birth injections are given to result in no ovulation events so
that pregnancy cannot occur. Type of injections KB such as Depo
Provera and Depo Geston injected every 3 months since the first
injection, Noristerat administered every 2 months for the first
injections, CYCLOFEM is administered once every 1 month.
a) Advantages of injections of KB
(1) Easy to accept because injections have been known to the
community since long as treatment.
(2) Does not interfere with breast milk
(3) Reduce the risk of pelvic cavity infections
b) Lack
Injections of sleeping pills can be used by mothers who suffer
from breast cancer and genitals and mothers who suffer from
abnormal vaginal bleeding, suspected expectant mothers.
3) Male and female condoms
When compared to other conception tools, condoms are the most well-
known contraceptives, especially those devoted to men. However, it
has now circulated condoms for women; That is almost similar to a
male condom, but should be installed in the mouth of the vagina 8
hours before sexual intercourse.
a) Disadvantages: This type of contraceptive device can only be
used once, less effective in preventing pregnancy, can cause
discomfort to the genitals.
4) AKDR/IUD/Spiral
AKDR is a singakatnd Ari contraceptives in the uterus or also known
as IUD (Intra uterine Devices) and spirals. The AKDR is made of
plastic or copper, placed inside the uterus. AKDR prevents sperm and
ovum meetings. How to AKDR: AKDR inserted into the uterine
cavity by using insertel or tool for input, pplication of AKDR must be
given the type of AKDR installed for it, then notified time control,
The use of bias is taught to feel the AKDR yarn in the vagina at any
period after entering the vagina. If the client is not touched, should
return to the clinic, If the client experiences menstrual lag, abnormal
vaginal bleeding, abdominal pain and pain when contacting the doctor
a) Other advantages of the KB spiral are:
(1) Can be removed at any time, without affecting fertility.
Once released, your fertility can return to normal.
(2) Safe to use for nursing mothers.
(3) Reduce the risk of developing cervical cancer and
endometrial cancer.
(4) Do not make oils like birth control pills.
(5) IUD hormones can reduce the pain and cramps of PMS,
reduce blood alrants that are too heavy during
menstruation, and reduce the risk of ectopic pregnancy.
b) Lack
(1) Spiral position can be shifted, which can increase the risk
of hidden pregnant.
(2) Discomfort in the stomach when new spouse.
(3) For the first few months, you can experience blood spots
in irregular ways.
(4) Should not be used in women with inflammatory pelvic
disease or active sexually transmitted diseases; Conceive
or may be pregnant; Or have problems or illnesses related
to the uterus.
(5) Your menstruation is faster and is accompanied by cramps
when you use copper KB.
5) Implant
Implant contraceptives consist of 2 prosthesis tubes that contain
levenorgestrel hormones that can prevent very large pregnancies in the
same capacity as vasectomy and tubectomy.
a) Benefits of using implants
(1) After being installed for 3 years when using Norplant
(2) Does not interfere with breast milk
(3) Affects blood pressure
b) Lack
(1) Irregular menstruation
(2) Headache and nausea.
(3) Acne
