MA. MYRNA FERMO SORIANO, of legal age, Filipino, widow, and a resident of
Labo, Camarines Norte;

MYRIAN FERMO SORIANO, of legal age, Filipino Citizen, married to NOEL

AREVALO and a resident of Labo, Camarines Norte;

CRISTINA FERMO SORIANO, of legal age, Filipino Citizen, married to RAMON

SANTIAGO and a resident of Labo, Camarines Norte;

AILEEN FERMO SORIANO, of legal age, Filipino Citizen, single and a resident

ANTONIA FERMO SORIANO, of legal age, Filipino Citizen, single and a

resident of Labo, Camarines Norte;

WITNESETH:

That the AFFIANTS are the co-owner of a parcel of land covered and embrace
by TCT No. T-14482 identified as Lot 854-E, Psd-36040, located in Poblacion and
Bulhao, Labo, Camarines Norte;

That we caused the subdivision survey in order to end our co-ownership and to
have our separate Title and agreed that it be segregated as follows;

To: the Heirs of Antonio P. Soriano--- Lot 854-E-1 with an area of 16, 814 SQM

To: Theodore Horace T. Soriano ---Lot 854-E-2 with an area of 2,500 SQM,
Lot 854-E-3 with an area of 2,500 SQM
and Lot 854-E-4 with an area of 2,512 SQM

To: Ingrid Dominique T. Soriano--- Lot 854-E-5 with an area of 2,500 SQM
Lot 854-E-6 with an area of 2,500 SQM
Lot 854-E-7 with an area of 2,500 SQM
And Lot 854-E-8 with an area of 4,018 SQM

That we are all agreed of the plan (LRA) Psd-E2017000629 and confirmed to
the herein attached approved subdivision plan and technical description; thus, We do
hereby request the Office of the Register of Deed of Camarines Norte to issue a
separate of Transfer Certificate of Title in our names based from attached approved
subdivision plan and technical description, surveyed by Geodetic Engineer, Ariel L.
Racelis;
