RE: Permission to Conduct Research Study: Data Gathering

Dear Sr. Jennifer T. Abasolo, OP.

Good day! Blessed be God forever.

We are currently enrolled in the subject Inquiry, Investigation and Immersion Class
of Mr. Jose Solomon. We are currently in the process of writing our Research Study.
The study was entitled “Factors affecting the academic performance of junior high
school towards mathematics amidst Pandemic”. In this regard may we request to
conduct our Data Gathering Procedure on the said topic.

We are hoping that you will allow us to recruit Junior High School Student from the
school to anonymously complete a 4-section questionnaire through online using a
Google Form.

If approval is granted, the respondents will complete the survey using a Google
Form on April 30, 2021. The survey process should take no longer than 15 minutes.
The survey results will be pooled for the thesis project and individual results of this
study will remain absolutely confidential and anonymous. Should this study be
published, only pooled results will be documented. No costs will be incurred by
either your school/center or the individual respondents.

Your approval to conduct this study will be greatly appreciated.
