    I, Glenn Denver A. Adjarani a Grade 12 General Academic Strand student currently conducting a
    study entitled “Effect of Modern Technology to the Academic Performance and Teaching Strategy of
    Teachers in Salvador Arollado Sr. Memorial High School”, in partial fulfillment of the requirements in
    Practical Research.

    In relation to this, may I respectfully ask for your permission to allow me to conduct a survey
    among [respondents]. The survey would last only about 5-10 minutes and would be arranged at a time
    convenient to the student’s schedule. I will highly appreciate the time you may allot to me upon
    conducting the above-said study.
