Patient Mario claimed that there are familial diseases that are existing in their family such as ulcer, asthma, and arthritis. He is the eldest child among 5 children. His parents are still alive and healthy according to the patient. The family experienced childhood illnesses such as measles, mumps and chicken pox. They also experienced common illnesses such as cough and colds, fever, headache, muscle ache and toothache. They managed it by using OTC drugs such as Biogesic for fever, Neozep for cold, and medicol for headache. They also use herbal medicine such as oregano. They boiled it, squeezed it then drink it once a day. They used it for cough and colds. Most of the family members only go to the hospital when they feel seriously ill.
II. PAST HEALTH HISTORY
Patient Mario is the eldest child among 5 children. He was born on August 23, 1989. He completed his vaccines or immunizations during his childhood and there is a scar of BCG vaccine in his right deltoid. During his childhood, he experienced common childhood illnesses such as chicken pox and it was managed by avoiding oily and salty foods and his mother boiled guava leaves and used the water for taking a bath to relieve itchiness; measles and it was managed by using black long sleeves and pajamas to cover up the whole body; mumps and it was managed by applying an “akot-akot” at the inflamed area to relieve the pain. He also experienced usual illnesses such as cough and colds, fever, and headache. For cough and colds, it is managed by taking Biogesic 500mg or Neozep 500mg and taken twice a day after meals. He also used to squeeze boiled oregano leaves and drink it once day. For fever and headache, it is managed by taking Biogesic 500mg or Medicol 500mg and taken twice a day after meals. Patient Mario does not smoke neither drink alcoholic beverages. He also claimed that he has no allergies on foods or medicines.
III. PRESENT HEALTH HISTORY
On June 7, 2021 @ around 5:05 in the morning, he was rushed to Provincial Hospital due to stage 2 burn wounds on his anterior and posterior torso and entire right arm and right leg with stage 1 burns on his anterior neck. Few minutes prior to admission, Mang Mario was cooking when the LPG tank exploded and caught on fire the LPG tank exploded and caught on fire. The entire house was engulfed in flames that causes his burn. Immediately after the incident, the fire department arrive, and he was rushed to the Hospital via ambulance. While enroute, the paramedics of Laoag City DPS had patient Mario with oxygen inhalation via nasal canula.
