USS Midway Museum
The longest-serving aircraft carrier in the world
has been permanently moored at San Diego’s
Embarcadero since 2004. Commissioned in 1945,
the USS Midway served in the Vietnam War and
Operation Desert Storm, before becoming the

20TH ANNIVERSARY
The Aflac Duck’s first commercial, “Park Bench,” aired on Jan. 1, 2000. It was
a hit. The Duck was an even bigger one. Since then, the Duck has become
an international celebrity, a world-renowned sales rep, a television star and an
advertising icon. Along the way, he helped boost Aflac’s awareness from 10%

evaluated each year to make sure they give you the cash and prizes you want most. Some of
these key changes include:

• First-year series is now only for associates/rookies with 52 weeks of


service or fewer.
• Second-year transition series will continue for associates within
Triple Crown
53-104 weeks during the 2019 calendar year (weeks 1-52).
• Higher payout amounts for new account bonus achievements.

• Eligibility for associates 53 weeks or greater.

The guidelines governing all Aflac Honor Clubs and Rewards are designed to give all associates and
coordinators an equal and fair opportunity to qualify for awards.

Associate Bonus
Qualification period:
Based on the associate’s contest start date.
• First-year series: Weeks 1-52.
• Second-year transition series: Weeks 53-104.

Eligibility:
New Associate/Rookie (Weeks 1-52):
• First-year series.
• Qualification is based on personal production of Aflac Traditional gross annualized
premium.
• The tiered no-pay rate for new associates only applies to the levels indicated within
the Triple Crown First-Year Series.

Second-Year Associate Transition Series (Weeks 53-104):


• Second-year series.
• Qualification is based on personal production of Aflac Traditional and Aflac Group
gross annualized premium.
• Only associates within 53-104 weeks during the 2019 calendar year (weeks 1-52) will
continue in the second year series until their 105th week.

Broker qualification (New single-producer brokers):


• First-year series.
• Only business processed at Level 01 will count toward contest qualification.

Promotion/demotion:
• All reinstated producers who had at least a one-year lapse in service will again be
eligible for the entire Triple Crown Series.
• Producers hired directly to DSCs or those producers promoted to DSC may
complete the entire Triple Crown Series but will not be eligible for the DSC New
Agent Success Bonus payout on themselves.
• Promotions from associate level to any level other than DSC are not allowed to
complete the entire Triple Crown Series.

Payment:
• Payments will be processed approximately 30 days after achievement.
• Payments must be reviewed and approved by Aflac Trust prior to payout.

Plaque/plate:
A plaque will be awarded the first time a series level of Fireball or higher is achieved. Each
subsequent achievement will result in an award of a plate for the level achieved to be attached
to the plaque.

New-account bonus:
• Both the annualized premium and the new-account requirements must be met prior to the close
of the qualification week in order to receive the new-account bonus.
• If an associate does not achieve the series level annualized premium requirements but achieves
the required new accounts, a new-account bonus will not be paid.
• If an associate achieves the annualized premium requirement but fails to achieve the new-
account requirement, they will still receive the annualized premium bonus payout and awards
for that level.
• Series-level qualification will be earned based on annualized premium and no-pay requirements.
• For contest purposes, new account refers to any account that meets all the new-account
guidelines established, including:
-- Three or more unique policyholders with new policies issued.
-- Existing policies that are transferred to the account will not fulfill the policies issued criteria.
-- Sub-accounts (those linked to master accounts) will not provide credit for this criteria.
-- Only the associate listed as the original servicing agent will be credited as the account opener.

All levels in Triple Crown are based on the contest start date, also called the entry
week. The contest start date is established in one of two ways: 60 days from the
