CHAPTER I

PRELIMINARY

1.1 BACKROUND

Form has an important role brcause it can help facilitate administrative work
such as recording business transactions. In this life there are various kinds of
professions engaged in by the community to meet their needs. And one of them is
an administrations. And to become an administration must understand things in the
administrative world such as forms. Forms have an important role for companies
and are tools to facilitate administrative work.

Filling in forms can also train and improve our writing skills in the language we
want to learn for example English, do to practice it we must try to make and fill out
forms in English.

1.2 FORMULATION OF THE PROBLEM


1. What is Form?
2. How many type of the form?
3. How to fill in form?

DISCUSSION

2.1 WHAT IS THE FORM?

In the narrow sense, the form can be interpreted as proof of the transaction.
Or often also called a document.

In the broadest sense, the form is a slip that has been arranged in such a
format likeness to be filled in accordance with certain needs. Besides that, the form
too have printed information.

It can be concluded that the form is a sheet of paper or more which provide space
for loading notes, information or instructions will be forwarded to other individuals,
departments or organizations.

2.2 TYPE OF FORM

There are two types of forms, namely:

1. Internal forms:

forms used in an environment office between supervisors, employees, and a


representatives office space.

Examples: cost tickets, expense forms, factory orders, requests for materials, data
on sales, information regarding employee work results.

2. External forms:

forms used in an office environment which form is sent to customers. Examples:


purchase orders, invoices, credit memos.

2.3 CHARACTERISTICS OF A GOOD FORM

1. Have a clear identity.

2. The information content must be useful for the organization that uses it.

3. The element of filling information In accordance with the method of filling.

4. Information processing allows for the processing method used.

5. Can help form users.

6. Low cost

7. Easy follow up

2.4 MAIN PART OF THE FORM

The main parts, namely:

1. Introduction

The introduction is presented at the top of the form and must contain the title form
and form number.

2. Instruction

Instructions are of two types, namely how to fill out forms and what that must be
done on the form after completing.

3. Main contents (main body)

Logically related information must be grouped together on forms using columns


and square boundaries (boxes) which are used as much as possible to provide space
for data which is recorded.

4. Conclusions

Conclusions are presented at the bottom of the form. This part must provide enough
space to involve the final position and or the final approval of the transaction
recorded on the form including the mark approval hand and date.

2.5 HOW TO FILL IN FORM

WRITING LEGIBLY

Writing legibly on a form you fill in is vital. Every form is important so


make sure that the recipient of it does not have to spend time deciphering your
scrawl! It is a good idea to print most of the answers. If you are required to write
something, as in a car accident claim form, then remember to write legibly and
concisely.

• Don’t write the account of your accident straight on to the form.


• Write it out first and make sure it is clear and concise.
• Then copy it out neatly.

ASKING FOR HELP

Never be afraid to ask for help when filling in a form. Some forms can be
confusing but the company will be dealing with them all the time and will be only
too happy to help you. It is in their interests too. It is much better to explain
something to someone than to have to request that a second form be done as the
first one is incorrect.

Do take a few minutes before you fill in a form to read it carefully. This can
save you time later on. Make sure you fully understand it before you start to fill it
in. Of course, if you are doing it ‘online’, it is easy to delete errors.

WITNESSING THE SIGNATURE

Before your signature some forms put something like the following:

‘I declare that to the best of my knowledge and belief the statements above are true
and I have not withheld any relevant information.

It is also likely that you may be required to have your signature witnessed. This
means that you must let somebody watch you sign your name and then he or she
should sign underneath to say your signature has been ‘witnessed’.

CHECKING FOR ERRORS

• Always look through the form first and delete anything irrelevant.
• Write out any details in rough first.
• Write legibly.
• Ask for help.

PRACTISING WHAT YOU’VE LEARNT

• You wish to make direct debit payments to the Electricity Board. Fill out
the form on the previous page.
• Write out the details of a car accident for your insurance company.

CLOSING

3.1 CONCLUSION

A form is a sheet of paper or more that provides space to make notes,


information or instructions that will be forwarded to other individuals, departements
or organizaations. In this case, the form also has some provisions in making it and
has many benefits.

3.2 SUGGESTION

In writing this paper the author realizes that there are still many shirtcomings
and errors. Therefore, the authors highly expexct criticism and suggestions form
readers who are constructive for the perfection of this paper.

Praise us for the presence of Allah SWT, for the pleasure and grace we can
complete the preparation of a paper entitled FILLING IN FORM this well and on
time, we also thank the lecturer in writing who have given assignments about this
paper so that our knowledge in writing this paper is growing. There are no perfect
humans. Therefore, we realize there are still many mistakes that are accidentally
