I, PCpl Chuckie H Hila, male, legal age, member of Philippine National Police presently
assigned at Tactical Motorcycle Riding Unit (TMRU) Manila Police District (MPD) Police Station-
3 PS-3 after having been sworn to in accordance with law, do hereby depose and say:

1. That, the undersigned PNCO was directed by PMAJ JOSELITO P DE OCAMPO,

Summary Hearing Officer to explain in writing why the undersigned should not be
charged of SIMPLE NECLECT OF DUTY for Failure to report upon declaration of
alert status dated July 25-29, 2014 (July 25, 2014 at 6:00 pm, July 27, 2014 at 6:00
pm and July 29, 2014 at 6:00 pm);

2. That the undersigned PNCO on the said date and time was actually present and on
alert with in the vicinity of Camp Bagong Diwa during the taking of attendance from
July 25, 2014 to July 29, 2014. However on July 25, 2014 at 6:00 pm, July 26, 2014
at 6:00 pm and July 29, 2014 6:00 pm the undersigned PNCO was eating their early
dinner and unfortunately was marked absent;

3. That the undersigned has no intention of violating any directive and mandated task
and was actually one (1) of the recipient of Medalya ng papuri as a result of that
particular event. (Attached herewith a copy of Medalya ng papuri with General Order
Number 3266 as “ANNEX A”) ;

4. Further, request that the undersigned hoping from your good office a humanitarian
and considerate understanding regarding this matter and be exonerated from any
administrative case.

WHEREFORE, it is respectfully prayed unto this Honorable Office that the undersigned
be given consideration regarding this matter;

Other relief and equitable is likewise prayed for.
