JOINT AFFIDAVIT
(RE: LOSS)

We, LOURIE ANTONETTEE CHU a Sales Service Office (SSO) and

CIELO YPANTO a Sales Service Head (SSH) both of legal ages, Filipinos,
and both residents of Ormoc City, Leyte, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say that:

1. We are the officers in charge of a certain Encryption Keys of Cash

Acceptance Machine Bonifacio Branch and SM-Ormoc ATM Machine
under Leyte Ormoc Bonifacio Branch;

2. The abovementioned Encryption Keys was under our custody and

intensive care;

3. Sometime on December on the year 2018, it was accidently lost in

our possession and spite of diligent search and efforts, it could not
be found and the same is now beyond recovery;

4. The said lost was purely due to inadvertent the same;

5. We are executing this affidavit to attest the veracity of the

foregoing facts and for whatever other legal purpose this serves
best.
