PROJECT PROPOSAL

PROJECT TITLE

INNOVATIONS ON REGISTRY BARANGAY INHABITANTS (RBI) PROFILING

DESCRIPTION

This project is a paperless info gathering system that will truly help in all barangay planning. it
uses the Google Form. Google Form is free online software that allows us to create survey,
quizzes and other forms. It's part of Google's web-based apps suite, including Google Docs,
Google Sheets, Google Slides, and more. It's a versatile tool that can be used for various
applications, from gathering data or for an event to creating a pop quiz.

DATA COLLECTION

Pursuant to Section 394 (d) (6) of the Local Government Code of 1991, the barangay secretary
must keep a record of all inhabitants of the barangay. The record must contain the name,
address, place and date of birth, gender, civil status, citizenship and occupation. But, this
project will not only limit on the personal information but also it will also gather information on
one’s Health/Occupation/Education Information, Disaster/Hazard Information and Utilities
Information. In gathering data/information, data gatherer will use a Paperless Record of
Barangay Inhabitants Survey Form (PRBIS Form) thru Google Form with a mobile phone or
tablet. All the information that will be encoded on the BPRBI Form (Google Form) will directly
manifest to the Google Spreadsheet (Excel) – no need to encode the data like the old way. This
PRBIS Form is editable. Barangay can delete and add question/s that is/are not
applicable/applicable to the status and location of the barangay. Data gatherer will ask these
following information (based on the BPRBI Form of Barangay Motherlode):

This information is necessary in all Barangay Planning. This PRBI is a big help to the barangay
especially to all the barangay secretaries in complying reports and related info-gathering of
Municipal Offices. Age of the constituents will automatically update every year. Once the BPRBI
is already done, the Barangay Secretary will annually update the Spreadsheet – deceased or
additional family member.

Once all the fields are filled out, the data gatherer will click the submit button and the data will
automatically manifest to the Spreadsheet (Excel).

SUPPORT NEEDED

The Barangay Secretaries are asking the help of the Liga ng mga Barangay – Jose Panganiban
Chapter to materialized this project for the benefit of their owned beloved barangay. We would
like to conduct a Free Seminar-Workshop for the Barangay Secretaries on how to make the
Paperless Record of Barangay Inhabitants (PRBI) Survey Form and to learn the other functions
of this project. The said Free Seminar-Workshop will be facilitated by the Federation President,
Ree-Jay M. Mago, Barangay Secretary of Barangay Motherlode, pilot-user of the said survey
form.

EXPECTED OUTPUT

The Federation of Barangay Secretaries of Jose Panganiban is looking forward to a uniform and
reliable Recording of Barangay Inhabitants.
