SUBJECT : FINAL WARNING ON FOLLOWING MINIMUM HEALTH PROTOCOLS

First and foremost, we want to keep a safe workplace for everyone and protect the health of all
employees and at the same time minimizing the disruptions of our day-to-day operations. While
the management has done its share of supplying personal protection provisions such as face mask,
face shield, hand sanitizer/soap, alcohol, and Vitamin C supplement, it is up to the employees as
mature and responsible individuals to cooperate in taking stringent steps to prevent the spread of
the virus.

The recent infection of one of our colleagues and the subsequent quarantine of three others is a
consequence that we are not serious in following the health protocols. There are employees who
are not mindful of these health protocols such as avoiding close contact with fellow employees,
not wearing face mask properly, not reporting to HR/Immediate head when feeling sick.

Due to the blatant disregard, please consider this as a final warning to ALL EMPLOYEES to strictly
adhere to the minimum health protocols set by the management. Please be reminded that as
indicated in our Employee Handbook, “Disobeying the health protocols set by the
company/government”, shall have the following penalties:

First Offense – Written Warning

Second Offense – 1 to 10 Days Suspension
Third Offense – Dismissal

Continued disobedience shall be met with the above disciplinary actions. We hope this final
warning is met with great understanding and a positive response on your part.
