SUBJECT: GENERAL GUIDELINES ON LEAVE APPLICATIONS

It is the policy of the Company to provide its employees sufficient and reasonable periods of rest,
recuperation and time to attend to personal concerns. Leaves, however, shall be regulated and
subject to company rules and regulations.

All employees are entitled to six (6) day Vacation Leave upon regularization and (6) days Sick
Leave upon anniversary of employment. Unused sick leave will be monetized after a year.
Regular employees are entitled to five (5) days Bereavement leave with pay, while probationary
employees may take three (3) days bereavement leave with pay.

The following guidelines must be strictly observed upon application for leave:

1. All leaves must be properly authorized using the LEAVE APPLICATION form (LAF)
available at the HRS Office.
2. The approving authority for the leave application shall be the immediate supervisor/Unit
Head of the employee.
3. A sick/injured employee or representative should notify the immediate Supervisor/ Unit
Head immediately ON THE DAY OF HIS ABSENCE at least 2 hours prior to the shift
through any reasonable means for the latter to make certain adjustments if need be.
4. Sick leave must be filed the day after of availment. Sick leave in excess of four (4) days
requires a return-to-work note/medical certificate and should be submitted together with
the Leave of Absence (LOA) application form.
5. Bereavement Leaves may be availed of without formal request; however, a bereaved
employee or representative shall immediately inform his supervisor/Unit Head.
6. Vacation leave must be planned and filed ahead of time or at least three (3) days before
the intended availment.
7. Disapproved vacation leave if pursued by the employee will be considered AWOL and
will be dealt with accordingly.

8. Every employee who wishes to shorten or extend his/her approved leave should seek the
approval of his/her immediate supervisor/Unit Head and immediately notify the HRS
Office of such change. Failure to do so shall subject the employee to appropriate notice of
specific charge.

Note: The Management reserves the right to create, revise and repeal policies.

applicable following the company’s Code of Ethics and Discipline)
