JOINT AFFIDAVIT OF COHABITATION

          WE, ALBERT BAGAYANA CADANG and MARIA CECILIA MABRAS

BELLEN, both of legal age, Filipino, single (living together), and residents of
Villanueva St. Casile Brgy. 4-Quinale Paete, Laguna, after having been duly sworn to in
accordance with law, depose and state THAT:

1. We have been living together as husband and wife under the same roof,
continuously and without any interruption, since year 2000 or a period of
more than twenty (20) years;

2. During our cohabitation and even until the present, we remain both of single
status and hence, there exists no legal impediment for us to marry each other;

3. We are blessed with three (3) sons and one (1) daughter namely, JUSTIN
BELLEN who was born on August 25, 2001 in Sta. Cruz, Laguna,
QUIELBERT CADANG who was born on July 26, 2003 in Paete, Laguna,
ALEXANDRA GAILE CADANG who was born on April 10, 2010 in Paete,
Laguna and ALEX JAZZ BELLEN who was born on October 04, 2016 in
Pakil, Laguna;

4. We are executing this affidavit to attest to truth of the foregoing facts and for
the purpose of contracting marriage without the need of securing a marriage
license pursuant to the provisions of Article 34 of the Family Code of the
Philippines. 

IN WITNESS WHEREOF, we have hereunto set our hands this 16th day of
January 2021 in Paete, Laguna, Philippines.
