I, DAVID L. DUMALUAN IV of legal age, Filipino and a resident of Brgy. Bolod


Panglao, Bohol, under oath, hereby state:
1. That I am a grantee of a CPC to operate a TOURIST CAR TRANSPORT service
under LTFRB Case No. 07-16-0849 on the line WITHIN TAGBILARAN CITY
AND TO ANY POINT IN THE CITIES AND PROVINCE OF REGION VII, with
the use of Three (3) units, particularly described as follows:

unit should be consolidated and made part in a single juridical entity and that my
individual CPC will be voluntarily dissolved to give way to its consolidation.

4. That I am aware that the failure to consolidate my CPC will result in the dissolution
of my CPC and the opening of the routes to applications filed by qualified juridical
entities such transport cooperatives and corporations.

5. That I attest the correctness of the foregoing statements.
