The BS Nursing Level II Section B students of the College of Health Sciences of this
University are currently enrolled in NCM 109b. One of the requirements for the course is to
present a case analysis of a patient suffering from pediatric condition.

In connection to this, the members of Group II as represented by the undersigned, would

like to seek permission to select one (1) patient who has the above condition and conduct an
interview and a thorough clinical assessment regarding the patient’s health status.

Moreover, I would like to request that the group to be allowed to review the chart of the
patient with whom we are to conduct the interview and assessment and to appraise the patient’s
condition for three days from the day of initial assessment. Rest assured that the identity of the
patient will be kept confidential and all information gathered will be used for academic purposes
only.
