Overview

• Access: Entry points to a local housing and service system, which includes
physical front doors, outreach teams, hotline, virtual access, after hours
access, etc.
• Assessment: Standardized triage and assessment process that is adopted
across the CoC and intended to increase consistency and fairness.
• Assignment: Process of referring households experiencing homelessness to
housing and/or service openings, utilizing CoC prioritization policy.
• Accountability: Outcomes and measurements that enable a CoC to know if
stakeholders are meeting expectations, and if the coordinated entry system
is effective.

Screening Tool
• Vulnerability Assessment Tool (VAT)
• Created by Downtown Emergency Services Center (DESC).
• 10 domains, Each serves as a question for a total of 10 questions.
• Likert scales and screener can add comments.
• Items are summed for total score.
• No cut offs provided, those with highest score are considered highest risk.
• Requires training and limited assessors to maintain the fidelity of the tool.

Purpose:
• Meet HUD requirements, national best practices.
• Decrease confusion for individuals experiencing homelessness.
• Increase reliability and validity, ensuring most vulnerable are prioritized.
Process:
• Began exploring where majority of individuals are already going.
• Aware this may change over time, pending information from pre-screen.

Committee members will present updates at each monthly GKCCEH Meeting. Additional training and
information sessions may be developed.
Following each monthly GKCCEH Meeting, members of the Coordinated Entry Work Group will be available
to answer specific questions.
