Questionnaire for 4/E

A. Bunkering

1.What are the preparations/checks required 1-2 days prior bunkering.

2.What are the checks you will carry put prior starting of the bunkering operation.

3.What are the check you will carry out just after the bunkering has started & subsequently thereafter
for the rest of the operation.

4.What will be your reaction once a spill is observed by you.

5.How frequently will you take the sample of the oil being bunkered.

6.How frequently will you be taking the soundings of the tanks which are bunkered & which are not
bunkered.

7.Where do you store the bunker sample bottles.

8.How much comingling is allowed with the present ROB of the bunker.

9.What precautions to take prior the air is blown from the shore/barge side.

10.How do you calculate the quantity bunkered, what is V.C.F

11.When 2 tanks which you are bunkering to 90% capacity are filling up to the capacity,what will be
your plan for the completion.

12.How many bunker samples are required to be taken.

13.Do you measure the H2S content of oil you are bunkering,what is the maximum permissible limit for
it.

B. Survey/Inspection/Audits

1. Are you aware of CMS, what is its significance?

2. What all items are checked during a Boiler annual survey?

3. How will you prepare the machinery under your direct charge for the upcoming vetting inspection?

4. Why & who carries out the vetting inspections.

5. What all are the items most likely to be inspected by the PSC inspectors during their inspection.
6. How will you prepare for the ISM audit?

7. What is a NCR & how do you close it.

8. How important it is to maintain your correct rest/work hrs.

9. Why & who conducts the ISM audit.

C. Emergency Preparedness/Safety Awareness

1. What will be your reaction when you see the smoke coming out of the purifier room or A/E or or from
one of the unit in M/E.

2. What will be your reaction when you are on watch in the E/R along with the oiler & you hear the fire
alarm?

3. What is the hot work policy of your present company?

4. When 2/E ask you to enter the M/E scavenge space for inspection, how will you go about it.

5. What are the checks you will carry out before entering the bilge tank?

6. What will be your reaction when you are on watch & you get the M/E scavenge fire alarm.

7. What will be your reaction while you are on watch & you get M/E high oil mist alarm?

8. What procedure you will follow, when you want to turn the M/E at anchorage or in port for
maintenance or for blowing through.

9. You are about to do the major o’haul on purifier/A/E/Pump/compressor(any of these m/cries),what


procedure you will follow prior starting the job.

10.Have you heard of tool box meeting, what is its significance.

11.You were assigned a job during your watch,which you need to carry forward for your next
watch.What will be your action regarding this.

D.Technical Knowledge

1.Have you experienced the M/E decarb, what all readings/measurements are taken during a decarb.

2. During you watch, you find a thumping noise coming from one of the unit of M/E, what according to
you could be the reason.
3.What is the significance of the fuel p/p mark or load indicator on M/E.

4.What could be the probable cause of high exhaust tempratur on one of the M/E unit.

5.Have you o’hauled the M/E mountings, what all checks to carry out while o’hauling a fuel injector &
when o’hauling an exhaust v/v.

6.What all alarms & trips are there on the M/E.

7. How frequently the M/E fuel injectors are o’hauled.

8.How do you cut off one of the M/E unit in case of an emergency.

9.Have you experienced a A/E decarb, what all readings/measurements are noted down during the
decarb.

10.What are the test you carry out on A/E con rod during decarb.

12.How frequently are A/E con rod bolts are renewed. How will you know in use bolts are not be used
any more.

13.Do you know how to take tappets on A/E,how will you take tappets if all the marking on flywheel
have faded because of the age of the vsl.

14.Where &how do you carry out a dye penetrate test.

15. Pls explain practically with all its components, how a domestic fridge circuit on board works for the
meat room.

16.How will you come to know if there is air in the refrigeration system.

17.Why does a reefer compressor short cycles.

18.What is the function of TEV in a refrigeration system.

19.How do you charge gas in the refer system.

20.What all test you carry out boiler water & their significance.

21.A boiler is not firing at all,how will you do the trouble shooting.

22.What is surging & swelling in boiler.

23.What all alarms & trips are there on IGS,what is the setting of IG high temp trip.

24.What is the setting of a PV v/v & a PV breaker.

25.How & when do you carry out the IG blower wash,when do you steam blow the boiler uptake v/v for
IG.

E.Shipboard Operations

1.What are you duties as 3/E & 4/E, where can you find them.

2.How do you take over/handover watch at sea & at port while discharging.

3.Who is responsible for the safety of the junior/oiler in you watch.

4.What will be your action when you find your reliever is under the influence of alcohol.

5.which PMS system you have experienced so far in you career ,how important it is to maintain the
inventory of the spares.

6.what are critical spares.

7.What do you do, when you need some store item & its not there on board.

8.When do you raise the requisition of spares of the m/cry under your charge.

9.How important it is to reuse the rags where ever possible & not to waste the consumable store items.

10.What are Saturday routines,what all m/cries /items are checked during this routine.
