PROJECT PROPOSAL
COMMUNITY HEALTH NURSING LEVEL IV
I. Project Title
“AKSYON IWAS ALTAPRESYON”: THE PREVENTION AND MANAGEMENT OF
HYPERTENSION IN OLDER ADULT AND ELDERLY CONSTITUENTS OF CATIAOAN
GASWILING KAPANGAN, BENGUET
V. Project Beneficiaries
30-50 People
All individuals ages from 30 and above, male or female, with history of hypertension
and at risk of hypertension
VI. Project Background
The project hopes to broaden the community’s awareness of hypertension and how to improve their
health status by understanding and implementing means of prevention and management of the disease. In
connection with this, the project hopes to lessen the present incidence of chronic uncontrolled
hypertension in the community, especially within the older adults and the elderly constituents via public
health education and adherence to a healthy lifestyle of proper nutrition and diet as well as
implementation of regular exercise.
Last semester during our community duty we assessed & identified problems in the community of
Gaswiling and hypertension is the most prioritized problem identified in the community.
Most of the people in Gaswiling Kapangan are hypertensive especially those elderly & some of them are
the young adults, and some are the pregnant women. Due to lifestyle, and nutritional status.
Hypertension is very common in the CAR and most of the population of Gaswiling Kapangan Benguet
are hypertensive and it has a big impact to their lives if we can’t find a ways to prevent this. Hypentesion
also can affect their over all body by experiencing stroke, heart attack and other complicated problems
and it can affect the way they live
Hypertension is common among the late middle aged and elderly constituents of Gaswiling Kapangan.
Based on the national survey released by the Department of Health (2017), more than twelve (12) million
Filipinos suffer from hypertension with half of this population unaware of their condition and it was
ranked the highest (38.6%), followed by stroke and CAD, in a study analyzing incidence of
cardiovascular diseases in the Philippines (Sison, Divinagracia, et. al., 2020). Hypertension is a chronic
non-communicable disease that frequently goes unnoticed because of its often asymptomatic nature,
nicknamed the ‘silent killer’ and therefore remains unmanaged by hypertensive individuals, risking
complications. Dr. Morales, President of the Philippines Society of Hypertension (PSH) states, “Most of
the time, hypertension has no symptoms…This makes the condition more dangerous. If it remains
untreated, the arteries and other vital organs in the body will be damaged”(Agron, 2012). According to
Mayo Clinic, uncontrolled blood pressure o]en leads to complications such as heart attack or stroke,
aneurysm, heart failure, and other peripheral vascular diseases (Sheps, 2018).
The specific objective of this “AKSYON IWAS ALTAPRESYON”: THE PREVENTION AND
MANAGEMENT OF HYPERTENSION IN OLDER ADULT AND ELDERLY CONSTITUENTS
OF CATIAOAN GASWILING KAPANGAN, BENGUET is to eliminate complications derived
from hypertension
Actual implementation of this proposed project will face definite setbacks, especially those that may
concern lack of audience participants or preparation of the actual event beforehand as well as technical
difficulties that may occur with equipment in the actual event. Nonetheless, the proposed project will
make use of present means to minimize these potential challenges. The number of audience members will
vary depending on how promoted the program is before the actual event of the first day of the program.
To address this potential challenge, the project must gain the support of Barangay organizations
pertaining to supporting health care, specifically the promotion of health of elderly and older adults.
