III. Scope of Work:

1. Supply of one (1) lot - CCTV Surveillance system including labor and materials such as - BNC
connector, Video balun, CAT 5 e cable, connector, locknut & bushing, adaptor and EMT pipes.
2. Dismantling and installation of existing CCTV and cable outside bank perimeter
3. Supply and installation of one (1) unit - UPS equivalent capacity of the CCTV system
4. Supply of platform and hangers.
5, Supply of O&M manual and brochure
6. One (1) week Training and Seminar
7. Testing and commissioning

IV. Delivery/Installation Site/Completion Period

V. Conditions of the Contract:

1. The Supplier/Contractor shall ensure that the items delivered are in accordance with the
specifications required.
2. The Supplier/Contractor, in consideration of the payment to be made by DBP of the sum
of money hereinafter specified, binds itself to pay, furnish, install and complete any and
all necessary permits, materials, labor, tools, equipment, supplies, utilities,
transportation, superintendence, supervision and other facilities for the satisfactory
performance for the supply installation, delivery, testing and commissioning of One (1)
Lot CCTV Surveillance System at DBP Palayan Branch.
3. The Supplier/Contractor shall be fully responsible for the safety of his men and clearly
understand and agree that no employer – employee relationship shall exist between the
Supplier/Contractor’s men and DBP. Any damaged caused by the Contractor or its
personnel to any property of DBP or injury or death to a third party shall be the sole
responsibility of the Supplier/Contractor and the latter shall pay the cost or shoulder the
burden thereof.
4. The DBP shall be free from any kind of claims, damages, liabilities or courses of action in
the event the Supplier/Contractor violates any terms and conditions hereof, including
any and all liabilities arising from the Supplier/Contractor’s non – compliance with the
requirements of the Workmen’s Compensation, Social Security and other labor laws.
VI. Warranty Period must be covered by Certificate

➢ One (1) year warranty upon final acceptance of the project. If any of the Office Machine
Equipment developed trouble and or malfunction within the warranty period the same shall be
repair/parts replaced free of charge and its warranty will be extended until such time the
contractor completed the repair.
➢ Cost of transportation and technicians per diem shall be also to the account of the contractor.

VII. Payment:

1. Payment shall be processed after completion of delivery subject to complete documents for
payment.
2. For every day of delay, 1/10 of 1% of unperformed/undelivered quantity shall be deducted
from the payment, in case the Supplier/Contractor fails to deliver/complete the project/items on
time.

VIII. Interested Supplier/Contractors must submit the following:

a) Proposal/Quotation
b) Valid/current PhilGEPS Certification with Registration Number
c) Valid/Current Business Permit
d) Notarized Omnibus Sworn Statement
e) Brochure of the exact brand/model/number of CCTV surveillance being offered.
f) Delivery Receipt
g) Warranty Certificate
h) Certificate of Completion/Acceptance issued by DBP
