The Cause and Effect diagram (also known as the Fishbone Diagram) is an analysis
tool you can use to:
Categorize many potential causes of a problem or issue in an orderly way.
Analyze what is really happening in a process
Teach teams and individuals about current or new processes and procedures
In the next example, an employee in an accounting department uses the Cause and
Effect Diagram to get to the “root cause” of a problem.
Note: Recorder does not necessarily decide which category the potential cause belongs in - that is a group
decision.
The effect refers to the issue (problem or process condition) you are trying to change.
Write the effect in the box on the right side of the Cause and effect Diagram.
The diagonal lines that “branch” off the main horizontal line of the Cause
and Effect Diagram are called major cause categories. You can use major
cause categories to organize the causes in a way that makes the most
sense for your specific situation.
Remember, these categories are only suggestions: you may use any category that
helps you to organize your ideas.
At this point, look for causes that appear in more than one category. This is
an indication of a “most likely cause”. Circle the most likely causes on the
diagram (see diagram-3.5)
Review the causes that that you have circled (the most likely cause) and
ask, “Why is this a cause?” Asking “why?” will help you get to the root cause
of the problem.
Record the answers to your “why?” questions on a separate sheet of flip
chart paper.
After you narrow down the most likely causes, choose from that group those
you feel are the most probable causes.
