Intermittent Pneumatic Compression

Evaluation:
1. Vital signs (Most important one is Blood Pressure)
2. Arm Girth
3. Sensory assessment (most important, pressure) *not needed for PT1 just yet

(Optional Topic: Sensory Testing)


- to ask Px if sense is intact for treatment proper
> Light touch: cotton
> Pain: paper clip
> Temperature: hot or cold test tube
> Pressure: thumb pad ------------------------ important for IPC
> First, test with OPEN EYS
 then test with CLOSED EYES
*do on one side then perform on other side, then compare results

Girth (Anthropometric) Measurement


- use eyeliner or anything that is easily removed from px’s skin. Hypo-allergenic.
For UE
Landmark: Lateral Epicondyle (mark with pen) Lateral Epicondyle
1. Measure 2in above initial mark, then mark.
2. Measure 2in above previous mark, then mark.
3. Go back to landmark, measure 2in below, then mark.
4. Measure 2in below previous mark, then mark.
Shortcut: 4 inches above landmark, 4 inches below landmark
*for CIRCUMFERENCE: (Use centimetres)
- Go to each mark and get the circumference at each point (start with landmark, go up then go down)

For LE (Px in long sitting)


Landmark: Medial Tibial Plateau (mark with pen)

1. Measure 2in above initial mark, then mark.
2. Measure 2in above previous mark, then mark.
3. Measure 2in above 2nd previous mark, then mark.
4. Go back to landmark, measure 2in below, then mark.
5. Measure 2in below previous mark, then mark.
6. Measure 2in below 2nd previous mark, then mark.
Shortcut: 6 inches above landmark, 6 inches below landmark
*for CIRCUMFERENCE: Same process as UE (Use centimetres)

For Foot (wasn’t able to get much notes on this one, sorry)
*for CIRCUMFERENCE (Non-volumetric measurement)
1. Start at mid-foot (level of 2nd toe), go lateral (towards pinky toe), under foot, then up medially to starting point,
wrapping the foot.
2. Once at starting point, go lateral around malleolus, behind Achilles tendon, medial then back to starting point.

1. 7000 (non-sequential; 2 holes)
2. 7500 (sequential; 3 holes)

*when high, follow outer inflation/deflation circle values

4 sleeves: foot, LE, Arm, hand

- Treatment time is 20 minutes. (Manual timing is done.)


- Check for BP at required times.

For UE
- limb should be higher than heart and keep limb straight
- 40 – 60 mmHg
1. Cover limb w/ pillowcase
2. Apply sleeve then stick tube to sleeve
3. Turn machine on
4. Set machine pressure to desired value (for this example: 60 mmHg)
5. Initially set inflation time to max and deflation at 0. Wait for gauge to reach desired pressure (in this case,
60mmHg)
*Remember to always ask feeback from patient
6. Once pressure reaches desired value, set inflation and deflation time to real settings (3:1; i.e. 60 sec: 20 sec)
7. Once treatment is done, set knobs to 0, remove sleeve, check skin integrity of px, and deflate sleeve
8. Check VS of px again.

For LE
- same process as UE
- 60-80 mmHg
On/Off switch
Non-sequential KCI ( 7000, 4 allowed sleeves)  Use BLUE TUBE

For LE
- limb is elevated and px is supine
1. Cover limb w/ pillowcase and place appropriate sleeve
2. Attach tube to sleeve & turn machine on (set dial to MAX)
3. Find the hole where the air comes out and place other end of tube there
- air comes out of either hole at intervals, so just place tube where air goes out and repeat until desired
pressure is reached (i.e. 70mmHg)
- first inflation; then lower pressure to maintain at desired pressure (in this case, 70mmHg)
4. Once consistent desired pressure is reached, start timer for treatment (20mins.)
5. Once treatment is done, set knobs to 0, remove sleeve, check skin integrity of px, and deflate sleeve
6. Check VS of px again.

Sequential KCI (7500, 2 sleeves: LE and UE)

* 3 tubes; color-coded (blue, red, white)


1. Cover limb w/ pillowcase and place appropriate sleeve
2. Attach tubes to sleeve & turn machine on (set dial to MAX)
- attach tubes to appropriate color-coded holes
3. Wait for full inflation of distal area of sleeve before lowering pressure knob to appropriate pressure.
4. Once consistent desired pressure is reached, start timer for treatment (20mins.)
5. Once treatment is done, set knobs to 0, remove sleeve, check skin integrity of px, and deflate sleeve
6. Check VS of px again.
