1 Purpose
1.1 To define the guidelines for performing hot works.
1.2 To ensure employee safety and prevent serious injury or fatality while performing hot
works

2 Training Requirements
The following employees shall be trained on this guideline:

2.1 Maintenance Manager, Supervisors, Foreman, Technicians, Helpers, and Staff


2.2 Services Manager, Supervisors, Foreman, Mechanics, Electricians, and Staff
2.3 Fabrication Manager, Supervisors, Foreman, Mechanics, Electricians, and Staff
2.4 Production and Plant Manager, Supervisors, Foreman, Operators, Helpers, and Staff
2.5 Materials and Logistic Manager, Supervisors, Coordinators, Foreman, and Drivers
2.6 Technical Manager, QA/QC Engineers, Laboratory Supervisors, Foreman, and
Technicians
2.7 QHSE Manager, HSE Officers, and Security Officers

3 Guidelines

3.1 Before the Activity


3.1.1 The Foreman or Welder shall inform the HSE Officer of the task involving hot works, i.e.
welding, grinding or cutting.
3.1.2 Request for the Hot Work Permit from the HSE Officer.
3.1.3 Perform the precautions listed in the permit, i.e.:
i. Notify all concerned personnel of the planned activity
ii. Specify the specific hot work to be done
iii. Confirm which of the following will be used: acetylene, oxygen, or welding set
iv. Identify the personnel to perform the hot work and the stand-by fire watchman
v. Provide the necessary PPE, e.g. welding gloves, apron, safety goggles, mask, etc.
vi. Keep a fire extinguisher or fire hose on stand-by
vii. Barricade the area where hot work will be carried out and provide appropriate
warning signs
viii. Provide adequate lighting and ventilation

ix. Cover all floor or pipe openings


x. Verify that the gas hoses are in good condition
xi. Keep the area away from heat ignition sources or activities, e.g. smoking, open
lights, torch, flames, etc.
xii. Clear the work and adjacent areas of combustible or flammable materials
xiii. Perform lock-out tag-out (LOTO) if applicable – Refer to GCSI-HSE-OPC-004
xiv. If applicable, verify that the Pre-Task Checklist for cleaning central mixer is
completed
xv. Determine any other necessary precautions based on the nature of the activity
3.1.4 Seek consent from the Supervisor then submit the filled-out permit to the HSE Officer.
3.1.5 The HSE Officer shall confirm that the above precautions are completed. If approved, the
name or badge nos. of the personnel involved shall be included in the form. Otherwise,
the reason for declining the request shall be specified.
3.1.6 Upon approval, request for a copy of the form to be made available in the designated
work area.
3.1.7 In case the hot work will be carried out in an enclosed space, secure the Confined Space
Entry Permit and follow GCSI-HSE-OPC-005 Confined Space Entry. Gas test shall be
performed and results shall be recorded.
3.1.8 In case the hot work will be carried out at 1.8m or above, secure the Working at Height
Permit and follow GCSI-HSE-OPC-003 Working at Height.

3.2 During the Activity


3.2.1 Ensure that the stand-by fire watch man has no other task to avoid distraction. Do not
leave the work area without replacement.
3.2.2 Steer clear of falling molten slag.
3.2.3 Consult with the HSE Officer as often as necessary to ensure safety while performing the
activity.

3.3 After the Activity


3.4 Clear the work area, remove any used tools, and ensure that it is left safe and free from
hazards.
3.5 Inform the HSE Officer once the activity is completed. The HSE Officer shall verify the
above and note this in the permit.
3.6 In case of an incident or accident, include the Incident Investigation Report reference no.
in the permit.
