1 Purpose
1.1 To prevent the unexpected energization, start-up or release of hazardous energy of
equipment and machines
1.2 To ensure employee safety and prevent serious injury or fatality in the workplace.

2 Training Requirements
The following employees shall be trained on this guideline:

2.1 Production and Plant Manager, Supervisors, Foreman, Operators, Helpers, and Staff
2.2 Maintenance Manager, Supervisors, Technicians, Helpers, and Staff
2.3 Workshop Manager, Supervisors, Foreman, Mechanics, Electricians, and Staff
2.4 Materials and Logistic Manager, Supervisors, Coordinators, Foreman, and Drivers
2.5 Technical Manager, QA/QC Engineers, Laboratory Supervisors, Foreman, and
Technicians
2.6 QHSE Manager, HSE Officers, and Security Officers

3 Guidelines
3.1 Identify the types of energy sources used, potential hazards and all control devices.
i. Electrical
ii. Mechanical
iii. Thermal
iv. Chemical
v. Gravity
vi. Hydraulic/Pneumatic
vii. Rotational Energy

3.2 Prepare all the materials required for the lock-out tag-out (LOTO) procedure as follows:
3.2.1 Materials: Lock Out / Tag Out devices complete set (locks and tags)
3.2.2 Equipment and Tools
i. Approved Tool box kit
ii. Digital Multi meter
3.2.3 Safety Equipment
i. Work gloves if necessary (voltage rated gloves if working near exposed voltage
must be greater than the rated working voltage)
ii. Safety helmet, high visibility vest, Safety boots or shoes

3.3 Inform the HSE Officer of the task involving LOTO and request for the permit. Refer to
GCSI-F-HSE-53 Lock-out Tag-out (LOTO) Permit.
3.4 Request from HSE Officer or Security for the LOTO equipment with the LOTO tag
bearing the requestor’s name, employee number, designation, and contact number. The
HSE Officer shall log the request.

3.5 Write the following information on the LOTO tag: the planned activity, date, and time.
3.6 Notify all affected employees of the specific activity.
3.7 Turn-off all operating controls.
3.8 Locate all energy sources.
3.9 Isolate all energy sources by blocking, bleeding and venting stored energy as found in
springs, hydraulic systems and pneumatic systems.
3.10 Lock-out all switches and energy controls in the “off” or “safe” position.

3.11 Test the operating controls by putting all controls in the “on” position. Ensure that the
work area is clear and nobody may get hurt before testing.
3.12 Return all operating controls to the “off” position.
3.13 The Permit must be posted at site until completion / cancellation of work.
3.14 Perform the planned activity.
3.15 Upon completion of the activity, check the work area again to ensure no one is exposed
to the hazards.

3.16 Once clear, remove lock-out devices only after the equipment is fully assembled and all
affected employees have been notified. Each lock-out device must be removed by the
person who puts it on.

3.17 Return the LOTO equipment and tag to HSE Officer or Security.
3.18 In case lock-out was not removed by the person-in-charge before leaving work, his
immediate superior can remove the lock-out only after ensuring that it is safe to remove
it. It is necessary to contact the person who puts the lock-out.
3.19 Do not force the removal of the lock-out. A corresponding sanction shall be enforced
according to the circular on HSE Violations Penalty System.
3.20 Ensure to close out the permit and return back to HSE Officer for archiving.
