1. PURPOSE:

The purpose of this management system is to protect employees from energy


source through a logic sequence of steps to drive to a safe
equipment/machinery installation, inspection, test, maintenance and operation
in a safe manner, avoiding loss of containment and unnecessary risk exposure.

2. SCOPE:

This management system applies to all employees and contractors within


Petrokemya premises for the control of all relevant energy sources during
servicing, maintenance and or installation of machinery and/or process
equipment, when:
2.1 Required to remove or bypass a guard or other safety device.
2.2 Required to place any part of their body into an area on a machine or
process equipment where work is actually performed upon the material
being processed (point of operation).
2.3 An associated danger zone for potential energy exposure exists during a
machine operating cycle.
2.4 There is a potential for a person to come in contact with such energy
sources that can potentially result in a negative consequence. An impact
on other EHSS disciplines takes place (spills, emissions releases, process
upset, etc.).

This is the physical test to start the equipment/process and open

5. PROCEDURE

5.1 Key Roles and Responsibilities involved in the LOTO Process


5.1.1 LOTO Coordinator/ Delegated person:
5.1.1.1 Shall be operation supervisor or operation specialist level or
above appointed by SMT. In case of area assign other than
operation similar equivalent level to be approved by SMT.
5.1.1.2 Shall attend LOTO authorization training provided by PK EHSS
department and pass the exam with 80% score as a minimum.

5.1.1.3 Shall be responsible for the LOTO program for their area of
responsibility (e.g. Site, unit, plant)
5.1.1.4 Is responsible for maintaining the integrity of the LOTO program
for their area of responsibility (Plant/Unit/..).
5.1.1.5 Is responsible for record LOTO used in LOTO Certification
Control Sheet (Attachment-11) for their area of responsibility
(Plant/Unit).
5.1.1.6 Has received the same training as a LOTO Authorized employee
in accordance with Training Requirements (Attachment-3).
5.1.1.7 Manage the keys in the LOTO Coordinator cabinet in such a way
that no unauthorized persons can obtain any key as these keys
control the safety of all persons working under LOTO with
regards to isolated energy sources in the plant/unit.
5.1.1.8 Has a clear overview of all ongoing LOTO activities within his
area of responsibility.
5.1.1.9 Is responsible for proper hand over and received back of all
LOTO activities after his working hours (Sufficient time will need
to be set aside to have an adequate handover of ongoing work
between the off-going and incoming assigned LOTO Coordinator
or area Owner Supervisor) using the LOTO Cabinet Hand Over
(Attachment-7)
5.1.1.10 Periodically verifies the LOTO program in the field to meet the
expectations of this program.
5.1.1.11 Shall consult with LOTO authorized person to de-LOTO
equipment or system, when work is finished.
5.1.1.12 Report concerns of all kind of dangerous (unsafe) situation by
issuing eSHEM report and communicated to Owner supervisor

5.1.2 LOTO Authorized Employee:


LOTO Authorized Employee is defined as an employee who:
5.1.2.1 Shall be qualified Sr. Operator / Technician, Lead Operator/
Technician or above. In case of area assign other than operation
similar equivalent level to be approved by senior management.
5.1.2.2 Shall attend the LOTO authorization training and pass the exam
with 80% score as a minimum.
5.1.2.3 Shall complete and pass the concern JQP area for operation and
JQP level for maintenance to be LOTO authorized.
5.1.2.4 Shall be nominated by concerned department Senior Manager
with experience more than 3 years in PETROKEMYA.

5.1.2.5 Has received training in the recognition of all potential applicable


hazardous energy sources, the type and magnitude of the
energy present in the workplace in accordance with Training
Requirements (Attachment-3).
5.1.2.6 Is authorized to Lock and or tags out machinery and/or process
equipment in order to perform service or maintenance work on
that machinery or process equipment.
5.1.2.7 Has received instruction on writing (Attachment-5: LOTO
Certificate) and deploying procedures for performing LOTO
activities and on the requirements for prohibition any attempts
to restart or re-energize machines or equipment under LOTO.
5.1.2.8 Has demonstrated a hands-on proficiency and the necessary
skills in applying LOTO in an adequate manner to the machinery
or process equipment that is being prepared for service and /or
maintenance.
5.1.2.9 Ensures that each LOTO point is isolated as per the LOTO
specific procedure and tags \ locks shall be registered in LOTO
certificate (Attachment-5)
5.1.2.10 Perform the tryout on the applied LOTO tools and locks.
5.1.2.11 Report concerns of all kind of dangerous (unsafe) situation by
issuing eSHEM report and communicated to Owner supervisor.
5.1.2.12 Shall validate LOTO Equipment Specific Procedures before they
are allowed to be used in the field.
5.1.2.13 Apply the general lock on the LOCK box after performing the
tryout.
5.1.3 LOTO Affected Person:
Defined as a person who is:
5.1.3.1 Required to work on machinery or process equipment under
Lock-Out/Tag-Out. LOTO Affected Person shall not be authorized
to perform the work unless and until that person places his
personal lock and tag on that equipment or on the LOTO box for
that equipment in accordance with the requirements of this Sub-
Element guarantee their individual control (maintenance,
operation, contractor, etc.).
5.1.3.2 Instructed about the intent of the procedure, and about the
prohibition relating to attempts to restart or re-energize
machines or equipment, which are under LOTO.
5.1.3.3 The executing person has the right to ask how the energy of the
equipment is secured via the LOTO verification point.
5.1.3.4 The executing person to ensure the proper isolation by try out as

per the LOTO specific procedure see Attachment-11.


5.1.3.5 Report concerns of all kind of dangerous (unsafe) situation by
issuing eSHEM report and communicated to Owner supervisor.
5.1.3.6 Shall attend the LOTO training provided by PK EHSS department.
5.1.4 LOTO Owner Isolator Person
Defined as a person who is:
5.1.4.1 Performing the isolation for the equipment\system to achieve
zero energy with applying owner lock (Red lock) and tag.
5.1.4.2 Shall complete and pass the concern JQP area for operation and
JQP level for maintenance to be authorized to perform the
isolation.
5.1.4.3 Shall attend LOTO training provided by PK EHSS department.
5.1.4.4 Instructed about the intent of the procedure, and about the
prohibition relating to attempts to restart or re-energize
machines or equipment, which are under LOTO.
5.1.4.5 Report concerns of all kind of dangerous (unsafe) situation by
issuing eSHEM report and communicated to Owner supervisor.
5.1.5 Owner Supervisor:
5.1.5.1 Shall attend the LOTO authorization training and pass the exam
with 80% score as a minimum.
5.1.5.2 Request initiator for isolation of the equipment by filling Section-
1 at LOTO certificate (Attachment-5).
5.1.5.3 Give the permission of equipment Temporary Release for
Testing if required by fill and approve of section-4 (Attachment-
5).
5.1.5.4 Shall Authorized the normalization of equipment after job
completion by filling of Section-5 (Attachment-5) – Closeout.
5.1.5.5 Verify the equipment has been checked, work completion
confirmed with work group and all related Work Permits are
close and equipment is ready for return back to service; in
addition to the (Attachment-5) (LOTO Certificate) closure
verification.
5.1.5.6 Control the Active LOTO Certificates in his plant/area.
5.2 General LOTO Requirements
5.2.1 To control releasing the energy associated with the service, maintenance
or installation of machinery and/or process equipment, there shall always
be a physical Lock-Out and Tag-Out device at each energy isolation
device applied by LOTO owner isolator person.

5.2.2 The LOTO equipment specific procedures shall be created by Area


engineer/ subject matter expert in case of SOP\SMP not meeting the
LOTO specific procedure requirements as per clause 5.4 and to be
reviewed by LOTO Authorized Person before performing the isolation
task.
5.2.3 The following elements to be included to ensure before any person
performs service maintenance or installation work on machinery or
process equipment where the unexpected energizing, startup or release
of (stored) energy could occur and cause injury, shall be isolated from
the energy source and rendered inoperative:
5.2.3.1 LOTO equipment specific procedures;
5.2.3.2 Employee and contractor training;
5.2.3.3 Periodic inspection program;
5.2.3.4 Assessment of Contractor LOTO Programs for alignment.

5.2.4 Energy isolating device(s) shall be locked out using the PETROKEMYA
appropriate lock out device(s) that shall be properly affixed to the source
of energy (machinery or process equipment) identified for installation,
service or maintenance.
5.2.5 As per this management system, energy isolating devices are further
defined as mechanical devices that when applied to machinery or
process equipment will physically prevent the transmission or release of
all forms of energy associated with the machinery and or process
equipment. Energy isolation devices include but are not limited to:
5.2.5.1 A manually operated electrical circuit breaker (only if in
compliance with IEC 60947-3)
5.2.5.2 A disconnect switch breaker (only if in compliance with IEC
60947-3)
5.2.5.3 An electrical plug;
5.2.5.4 Manually operated switches by which the conductors of a circuit
can be disconnected from all ungrounded supply conductors,
and, in addition, no pole can be operated independently. Only
switches that clearly indicate the state of isolation by an
approved “fail-proof” indicator or by the visible separation of
contacts (both as per IEC 60947-3) may be used to validate that
“Zero energy” is in place (other means of providing Zero energy
need to be described).
5.2.5.5 Line valves, flanges and any similar device used to block or
isolate energy.

Note: The push buttons, selector switches and other control


circuit devices are not considered as energy-isolating devices.
5.2.6 If an energy-isolating device is not capable of being locked out, the owner
shall conduct a formal JSA as per SHEM-02 and the action shall include
steps to utilize a Tag-Out only system, which still provides full employee
protection through additional control measures and an alternate LOTO
Equipment specific procedure capturing the additional control measures
that can achieve full employee protection shall be made.
5.2.7 The Tag-Out device shall be attached at the same location that the
lockout device would have been attached, and the employer shall
demonstrate the Tag-Out is conducted in accordance with the LOTO
equipment specific procedure and will provide a level of safety equivalent
to that obtained by using a lock and tag.
5.2.8 To demonstrate that the level of safety can be achieved when a Tag-Out
device can only be used, to control the release of energy, PETROKEMYA
shall include all additional elements necessary to provide the equivalent
protection that is provided when a lockout and Tag-Out device is used.
These measures can include:
5.2.8.1 Removal of an isolating circuit element (e.g. fuses);
5.2.8.2 Blocking of a controlling switch;
5.2.8.3 Utilizing an extra energy isolating device; or
5.2.8.4 The removal of a valve handle to reduce the likelihood of
inadvertent energization.
5.2.9 The owner shall make sure that machinery and process equipment are
design to accept energy isolation devices whenever required:
5.2.9.1 New machinery or process equipment is ordered and installed.
5.2.9.2 For older machinery and/or installations/process equipment
already in place at the facility, it shall be considered to update
them under the following conditions:
5.2.9.2.1 When replacement or major repair needs to be done.
5.2.9.2.2 When renovation or modification of machinery or
process equipment is going to be performed.
Basic LOTO Principle: One Lock, One Key, One Tag and One Person!
5.3 LOTO Equipment Specific Procedures
5.3.1 Area engineer/ subject matter expert shall develop written step by step
LOTO equipment specific procedures which shall be utilized in activities
to shutdown, isolate, block and /or secure machinery and or process
equipment to control hazardous energy see (Attachment-11).
5.3.2 All relevant energy sources shall be assessed for the risks they present

and these shall be documented in the LOTO equipment specific


procedure(s).
5.3.3 LOTO equipment specific procedures shall be approved by owner
department Senior Manager or Manager.
5.3.4 During Non- working hours, the shift Manager should be the approver.
5.3.5 LOTO specific procedures shall be developed for each machinery or
process equipment before maintenance and or service work can be
performed. The procedure shall include, but not limited to, the following:
5.3.5.1 The techniques which to be utilized for the control of hazardous
energy, such as electrical isolation, mechanical isolation …etc.
5.3.5.2 A specific statement of the intended use of the procedure;
5.3.5.3 Specific step by step requirements for shutting down, isolating,
blocking, drainage (depressurizes) and securing machines or
equipment to control hazardous energy.
5.3.5.4 Specific step by step requirements for the placement, removal
and transfer of LOTO devices and define the responsibility.
5.3.5.5 Specific step by step requirements for testing a machine or
equipment to determine and verify the effectiveness of LOTO
devices and other energy control measures to verify the zero
energy state of the machine or equipment, e.g. using multi-
meter to verify zero voltage, using start/stop push bottom to
verify the rotating equipment movement.
5.3.5.6 Specific step by step requirements that ensure all relevant
energy sources (including stored energy) are assessed.
5.3.5.7 Specific step by step requirements to DE-LOTO the equipment
back into service after work completion.
5.3.5.8 Define the LOTO points and LOTO Verifications/Confirmation
point (the testing processes to ensure the zero energy state) in
mark-up drawing and the drawing to be attached with LOTO
procedure.
Note: The existing SOP/SMP can be utilized as LOTO Specific Procedure if
it complies with LOTO Specific Procedure requirements (5.4) otherwise
the new procedure to be created.
5.3.6 The LOTO specific procedures shall be evaluated by LOTO Authorized
Employee prior to each use to ensure that they continue to provide
adequate worker protection (this is done to capture any possible
changes over time in the field). The deviations shall be identified and the
LOTO equipment specific procedures shall be updated accordingly.

5.4 LOTO Application


5.4.1 LOTO Coordinator shall maintain LOTO Certificate Control Sheet.
(Attachment-6).
5.4.2 LOTO Process:
5.4.2.1 Step 1: “Preparation for LOTO”
a. Make sure the availability of LOTO Equipment specific
procedure. (Refer to 4.4)
b. Review LOTO Equipment specific procedure before use. (Refer
to 4.4)
c. Fill and sing the LOTO Certificate before work execution.
5.4.2.2 Step 2: “Securing LOTO”
a. De-energize the Equipment/System according LOTO
Equipment specific procedure.
b. Perform LOTO of the equipment or system.
c. Control the execution of LOTO
d. Verify de-energizing.
5.4.2.3 Step 3: “Try out and verification”
a. Try-out by the person(s) who is going to work on equipment or
system.
b. Put personal lock on the LOTO box or satellite box.
c. Get the work permit.
d. Start working.
5.4.2.4 Step 4: “LOTO removal (Temporary Release or Work finished)”
a. If work is not finished, leave the general lock on the LOTO box.
b. When work is finished, remove personal lock from the LOTO
box.
c. Remove LOTO device according to LOTO Equipment specific
procedure.
d. If we need to reinstall the LOTO device after the temporary
release, go back to step 3.
5.4.3 Lockout/Tag-Out Device Application
5.4.3.1 LOTO devices shall only be placed on the machinery or process
equipment by LOTO authorized employees as defined by this
management system;
5.4.3.2 The LOTO device shall be registered in LOTO certificate and

control by Owner Supervisor.


5.4.3.3 LOTO devices, shall be affixed in a manner that holds the energy
isolating devices in a "safe" or "off" position;
5.4.3.4 When it is necessary to apply only a Tag-Out device, the tag shall
be affixed in a manner that clearly indicates that the operation or
movement of energy isolating devices from the "safe" or "off"
position is prohibited. Tag-Out devices shall be fastened at the
same point at which the lock out device would have been
attached;
5.4.3.5 Whenever a tag cannot be affixed directly to the energy isolating
device, the tag shall be located as close as safely possible to the
device, in a position that shall be immediately obvious to anyone
attempting to operate the device.
5.4.4 Stored Energy
5.4.4.1 Immediately after applying the LOTO devices to all required
energy isolating devices for the machinery or process
equipment, all potentially hazardous stored or residual energy
shall be relieved, drained, disconnected, restrained and
otherwise rendered safe by the authorized employee(s)
performing the work;
5.4.4.2 Whenever there is the possibility of re-accumulation of stored
energy, verification of isolation shall be continued until service
and or maintenance work is completed or until the potential of
accumulation is no longer present in the machinery or process
equipment.
5.4.5 The methods for verification after relies the stored energy shall be

5.4.6 Verification of Isolation
5.4.6.1 Prior to starting work on machinery or process equipment that is
under LOTO, the authorized employee who placed the device(s)
shall verify by testing the equipment that full isolation resulting
in a zero-energy state has been accomplished before beginning
the work.
CAUTION: Return Operating Controls to the
“Neutral” or “Off” Position after the Test.
5.4.6.2 In scenarios where a permit to work is required, work shall not
commence until all requirements of the work permit process
have been created, reviewed and approved by the persons
involved in the work task. Check LOTO Flow Chart (Attachment-1)
for full understanding of the LOTO process status.

5.4.7 Periodic Validation


5.4.7.1 If work extends over an extended period of time (multiple shifts,
days, weeks, etc.) LOTO device(s) can remain in place for the
duration of the work period. Continued control of the hazardous
energy must be verified by owner department supervisor at the
beginning of each subsequent shift or work task. Continuity of
the LOTO process must occur as shifts or personnel change.
5.4.7.2 This applies to personal LOTO types. The LOTO process shall
remain intact during these transition times by using direct LOTO
hand-off or Transition Locks and Tags.
5.4.8 Release from LOTO
Prior to the removal of LOTO devices from machinery or process
equipment and energy is restored, LOTO authorized persons shall
ensure:
5.4.8.1 The work area is inspected and that nonessential items have
been removed;
5.4.8.2 The machinery or process equipment components are
operationally intact;
5.4.8.3 The work area shall be checked to make sure everybody is safely
positioned or removed during the restart process;
5.4.8.4 That after the LOTO devices are removed and before the
machinery or process equipment is started, the LOTO authorized
person(s) shall notify all affected persons working in the area
that the LOTO device(s) have been removed and it is safe to
return to an operating state.
5.4.9 LOTO Device Removal
5.4.9.1 Normal LOTO Device Removal: Each lockout and/or Tag-Out
device shall be removed from each energy isolating device only
by the LOTO authorized employee who applied the device. In
case of absence of that Authorized person, the lockout and or
Tag-Out device(s) shall be removed by another LOTO Authorized
person familiar with the relevant installation after area owner
supervisor approval. This Authorized person shall verify the
Lockout and or Tag-Out device(s) are/is safe to remove and shall
follow the same LOTO Specific Procedure for the installation;
5.4.9.2 Emergency LOTO device removal:

The following steps shall be followed whenever the


employee who applied the lockout and/or Tag-Out device is
not available to remove LOTO devices or the lock key
missing or lost:
a) Shall be reported immediately & formally to owner, LOTO
Coordinator, executer supervisor, concern (executer)
department Sr. Manager and owner department Sr.
Manager. Verification by owner department Sr. Manager
including the LOTO coordinator that the employee who
applied the device is not at the facility.
b) The owner Supervisor has made reasonable efforts to
contact the employee who placed the LOTO device, to inform
him that their lockout and/or Tag-Out device has been
removed; (including requesting the employee to return to
remove the lockout/Tag-Out device(s)).
c) Confirm the work activity completed and all work permit and
certificate closed. “The LOTO certificate to be attached with
the Personal Lock Cutting Form (Attachment-4).
d) Confirm that no worker working on the isolated equipment.
e) Communicate to the affected area before lock cutting.
f) The cut out or damaged lock number shall be removed from
registration sheet.
g) The Lock Cutting Form (Attachment-4) shall be filled by LOTO
Coordinator / Owner supervisor and approved by concern Sr.
Manager and Operation Sr. Manager before cutting the lock.
Note: Unauthorized removal of a LOTO device or loss of lock out
device key shall be reported in e-SHEM as an EHSS incident (near
miss with high learning value team investigation requirement).
5.4.10 Below General, considerations shall be utilized for any draining or
proving before isolations commences:
5.4.10.1 Always drain to a safe location, including consideration of
volatility, temperature and composition of the material
5.4.10.2 Consider the arrangement / orientation of the system being
isolated and drained and ensure the drain is located as close to
the lowest point in the system as possible
5.4.10.3 If pressure indication is available within the isolations use this as
a secondary measure for determining if the system is drained
and de-pressured but take care to note that a de-pressured
system is not confirmation of a drained system.
5.4.10.4 Ensure adequate vents are provided where necessary during
draining.
5.4.10.5 Confirm there are no Work Permits live / issued in the adjacent

5.4.10.6 Use of correct standard of drain hose for service and ensure they
are secured at the drain point to prevent whipping under
pressure.
5.4.10.7 Always drain slowly when draining as high rates can cause static
sparking.
5.4.10.8 Never leave a drain open when unattended.
5.4.10.9 Any open end created or temporary hose fitted shall be
recorded in the appropriate register.
5.4.10.10 Inspect and verify that the equipment is de-energized after
draining is complete.
5.5 LOTO (Lock-Out/Tag-Out)
5.5.1 Whenever service or maintenance is performed on machinery or process
equipment it shall be done utilizing procedures that afford all employees
a level of protection equal to that provided by the implementation of
personal LOTO devices.
5.5.2 LOTO devices shall be used in accordance with the following steps:
5.5.2.1 The equipment\system qualified area owner shall put the owner
lock in the energy source (equipment/system) to isolate it.
5.5.2.2 The key of owner lock (Red Color) shall be maintained inside the
LOTO Lockbox (Movable or fixed).
5.5.2.3 The LOTO authorized (owner) shall verify that all LOTO
verification points are isolated as per LOTO specific procedure.
5.5.2.4 The LOTO authorized (owner) shall lock “Green” LOTO Lockbox
by the General lock.
Note: The above tasks can be handled by one person in case of qualified
area owner is LOTO authorized.
5.5.2.5 The key of General lock shall be maintained inside the LOTO Key
Cabinet (fixed). One LOTO Key Cabinet shall be provided for
LOTO Coordinator.
5.5.2.6 The LOTO Coordinator shall lock LOTO Key Cabinet by LOTO
Coordinator Lock. (Black color)
5.5.2.7 All LOTO affected persons shall lock LOTO Lockbox by their
personal locks (Yellow Color).
5.5.2.8 The satellite box can be utilized in additional to LOTO Lockbox if
there is extra group of workers from different disciplines work in
same isolated equipment or for workers work in the same group.
5.5.2.9 The LOTO Coordinator shall control and maintain the LOTO

5.5.2.10 The LOTO affected supervisor shall control and maintain the
satellite box.
5.5.2.11 The Lockbox number and satellite box number to be register in
LOTO Certificate (Attachment-5).
5.6 LOTO Exceptions
5.6.1 A combination of Lockout and Tag-Out shall always be applied when
work (maintenance, service...) with potentially hazardous energy sources
needs to be done unless one of the following conditions are applicable:
5.6.1.1 Exposure to hazardous energy is controlled completely by
unplugging the cord and plug connected equipment from an
electric outlet and where the person doing the service or
maintenance has exclusive control of the plug. This applies only if
electricity is the only form of hazardous energy to which persons
may be exposed when work is being performed by the person in
control of the plug (this exception encompasses many portable
hand tools and some cord and plug connected machinery and
equipment). Any activity, related materials, tools and equipment
are controlled by SHEM-08.10 requirements and controls.
5.6.1.2 Minor tool changes and adjustments, and other minor servicing
activities, which take place during normal production operations,
are not covered by this management system if they are routine,
repetitive, and integral to the use of the equipment for production,
provided that the work is performed using alternative control
measures that provide effective protection. These control
measures shall be prepared by person(s) familiar with the
equipment/process and reviewed and approved by LOTO
Authorized personnel or the department manager.
5.6.2 In situations where LOTO devices must be temporarily removed from the
energy isolating device and the machine or equipment temporarily
energized, e.g. for testing or positioning a machine, equipment,
installation or component thereof, the following sequence of actions
shall be followed:
a) Clear the machine and/or process equipment and adjacent
area of tools and materials by owner supervisor/ LOTO
authorized person.
b) Remove persons from the machine or equipment area by
owner supervisor/ LOTO authorized person.
c) Notify the area operations and affected personnel prior to the
temporary removal of locks/tags by owner supervisor/ LOTO
authorized person.

5.7 LOTO Devices and Tools
5.7.1 LOTO Devices
5.7.1.1 Locks shall indicate the identity of the permit receiver department
applying it. Following is the list of color for locks.

5.7.1.2 Locks & gang lockout (multi-lock hasps) devices shall be strong
enough to prevent removal without use of excessive force or
unusual techniques, such as with the use of bolt cutters or other
metal cutting tools.
5.7.1.3 The LOTO certificate is only an authorization for LOTO/equipment
isolation; but to perform the job, work permit is required.
5.7.1.4 Tags shall meet the requirements in accordance with General
EHSS Rules procedure (SHEM-8.01).
5.7.1.5 LOTO locks shall be unique from all other locks in use; and one
lock is issued with only one key for each person authorized in
LOTO practices to work on and/or be exposed to hazardous
energy during work practices.
NO spare keys shall be allowed under any
circumstance.
5.7.1.6 Lockout devices and Tag-Out devices shall be the only devices(s)
used for controlling energy, and shall not be used for other
purposes and shall meet the following requirements:

a) Lockout and Tag-Out devices shall be capable of withstanding


the environment to which they are exposed for the maximum
period of time that exposure is expected.
b) Tag-Out devices shall be constructed and printed so that
exposure to weather conditions or wet and damp locations will
not cause the tag to deteriorate or the message on the tag to
become illegible.
c) Tags including their means of attachment shall be substantial
enough to prevent inadvertent or accidental removal without
the use of excessive force; shall not deteriorate when used in
corrosive environments such as areas where acid and alkali
chemicals are handled and stored.
d) Locks shall be substantial enough to prevent removal without
the use of excessive force or unusual techniques.
e) Tag-Out device attachment means shall be of a non-reusable
type, attachable by hand, self-locking, and non-releasable with
a minimum unlocking strength and have the general design
and basic characteristics of being at least equivalent to a one-
piece, all environment-tolerant nylon cable tie.
f) LOTO devices shall indicate the identity of the person applying
the device(s) and the reason for its use.
g) Tag-Out devices shall warn against hazardous conditions if the
machine or equipment is energized and shall include a legend
such as the following: Do Not Start. Do Not Open. Do Not
Close. Do Not Energize. Do Not Operate.
5.7.1.7 Do Not Operate tags for system isolation will be as the followings:
a) Do Not Operate - Danger tags for field/process
isolation will be accordance to SHEM-8.01.
b) Refer to Specialized LOTO Isolation Tags
(Attachment-8).
5.7.1.8 Lockout devices shall be affixed in such a manner to prevent the
operation or movement of energy isolating devices from the
“safe” or “off” position.
5.7.1.9 If work extends over multiple days, owner and general lockout
device shall be remain in place for the duration of the work period.
However, when there is a change in the employees/contractors
continued control of the hazardous energy shall be verified to
ensure control of hazardous energy and sign the endorsements in
work permit.
5.7.1.10 Owner/Isolator LOTO Locks (Red Color):

An owner or isolator LOTO lock is a unique lock with one unique


key in combination with a general LOTO tag. It shall be used in the
isolation point at the field.
5.7.1.11 Authorized LOTO Locks (Green Color):
A general LOTO lock (LOTO authorized) is a unique lock with one
unique key in combination with a general LOTO tag. It shall be
used to lock the lock box only
5.7.1.12 Personal LOTO Locks (Yellow Color):
A personal and executer LOTO lock is a unique lock with one
unique key in combination with a personal LOTO tag. It shall be
used to lock the lock box or satellite box.
5.7.1.13 Transition Lock (Blue Color):
Single-keyed locks that indicates work on LOTO-applied
equipment has stopped temporarily and no one is currently
performing work on that equipment (e.g. Waiting for spare parts,
waiting out source service etc.) which can be placed on LOTO box
or on energy isolation device.
5.7.1.14 Transition Tag (DON’T OPERATE TAG):
Tag used in conjunction with a transition lock to indicate that no
one is currently performing work on that equipment see
(Attachment-9).
5.7.1.15 LOTO Tools
LOTO Lockbox. A lockbox is used if one or multiple persons need
to work simultaneously on the same equipment in order to create
the opportunity for all individuals to apply their personal lock and
tag on that lockbox and in that way individually control the keys
within that box.
Other LOTO tools include lockout stations, cables, chains, multi-
lock hasps and specific LOTO aids to help placing a lock and tag at
some locations.
5.8 Isolation Methodologies
5.8.1 Isolation: a set of measures taken to make and maintain safe a process,
an electrical or mechanical facility or an item of equipment, such that
energization of the equipment is not possible and energy/power cannot
re-circulate in it without deliberate action by all the authorized persons.
5.8.2 Mechanical Isolation is usually achieved by such means as blinding or
blanking, removal of pipe sections or spool pieces, double block and
bleed, blocking/ disconnecting all mechanical linkages. Energy sources
include electrical, mechanical, hydraulic, pneumatic (air), chemical,
thermal, radioactive, gravity…

5.8.3 Electrical Isolation


Key Consideration: Electrical equipment shall be considered to be
energized (i.e. live) until proven de-energized.
5.8.3.1 Steps in Isolating and Making Safe, the following key steps shall
be applied when making safe electrical installations:
a) Identify

c) Secure
d) Discharge (If Applicable)
e) Test
5.8.3.2 Only the electrical authorized person as per SHEM-8.05 shall
perform the electrical isolation.
5.8.4 Mechanical Isolation
5.8.4.1 Choice of the process Isolation
The following points are analyzed before choosing the type of
process isolation:
a) Information on the line and its content (pressure, inventory,
type of product);
b) History of passing valves;
c) Volume or inventory freed if the isolation barrier fails;
d) Presence of natural ventilation or confinement;
e) Number of exposed persons needed to perform the isolation;
f) Risks inherent in implementing the isolation, such as
breaking containment, critical lift, passing valves, etc.
5.8.4.2 The main isolation categories applicable to process isolations
are listed below in decreasing order of isolation integrity level:
a) Physical disconnection (such as the removal of a removable
spool piece);
b) Blinding/blanking/spading (spade, blind or plug, spectacle
blind);
c) Double block and bleed;
d) Double block;
e) Single block (and bleed).
5.8.5 Physical Disconnection

A piece of pipe (generally a removable spool piece) is removed from the


piping and a blinding device (usually a blind) is installed on the pressure
side, downstream or at the closest relief valve to wear the work is being
performed as well as the isolated process side. The separation
equipment has the same nominal characteristics as the piping.
On the isolated side of the system, blinds can be used to protect the
ends of isolated lines, if no ventilation of the system is required (see
Figure1) for an example). Rags are not to be used as protection because
they do not form an actual protection, can be forgotten and clog the
piping (valves and filters) when it is brought back into service.
Physical disconnection can be used for long-lasting isolations with any
type of fluids. This type of isolation is the preferred solution for vessel
and tank entry and all types of hot work. Hot Work activity requires that
the lines shall be blanked/blinded off or spaded on the process side of
the work.

5.8.5.1 Blinding/Blanking/Spading

Spading consists in installing a blinding device (spade or


equivalent, i.e. blind or plug, spectacle blind or plate) as closely
as possible to the equipment to be isolated (see Figure 2 for
examples). Blind selection to be as per Car seals& Blinds
procedure (SHEM 7.05).
A spade is an isolation device with a handle approximately 5 cm
(2 inches) or more that extends beyond the flange (see Figure 2
below). The characteristics of the spades could be marked /
stamped on the handle to identify the intended purpose of this
tool.
Blinding/Blanking/Spading can be used for a long-lasting
isolation with any type of fluid, for vessel and tank entry and for
all types of naked flame hot work when physical disconnection is
not possible.
The blinds/blanks/spades are installed as close as possible to
the equipment to be isolated and on each of its lines,
(instrument drain and venting lines included).
Special consideration is given to any change in piping class
when choosing the position of the blinds//blanks/spades, as
wrongly positioned blinds/blanks/spades can cause
overpressure in a system with a lower test pressure.

CAUTION: Keep in mind that historically the “identification


holes” to distinguish between the various types as shown above
can have a different meaning at different sites around the Globe.
Please use the correct pictures relevant to your site in your local
program (e.g., sometimes “orifices” are indicated with one hole
as well).
Only isolation valves are considered as appropriate means of
isolation. On actuated isolation valves, the energy supply (air,
gas, hydraulic fluid or electrical cable) has to be disconnected as
per the affiliate isolation procedure.
5.8.5.2 Double Block (Isolation) and Bleed
Double block and bleed isolation (as shown in Figure 3) is
provided by shutting two isolation valves and opening a valve
(bleed (drain, vent)) between them. The theory is that any
leakage past the upstream valve will come out via the bleed
(drain, vent) instead of leaking through to the downstream valve
(Control valves, trip valves or non-return valves are not isolation
valves). A potential risk is blockage of the bleed valve where (an
unobserved) pressure build up can occur and possibly leak
through to the second isolation valve and potentially into the
isolated process side.

5.8.5.3 Double Block:
If double block with bleed isolation is not possible, double block
isolation only could be acceptable, with the required
precautions, if backed by the risk assessment. Keep in mind that
this might not give a true indication if a full successful isolation
has been made. SAFER (SHEM-02) process may be necessary to
identify and mitigate the remaining risk.
5.8.5.4 Single block (Isolation)

A single block isolation is used only when there is no possible


alternative and the risk is acceptable, and is accompanied by
compensatory measures (see figure 4 for an example). The leak-
tightness of the single-valve isolation is demonstrated.

5.8.6 To be aligned with the forms, shall include an attachment of P&ID
drawing copy with all valves marked and numbered what to be opened
and what to be closed. Equipment isolation shall be reviewed and
approved by supervisor level for confined space entry isolation.
5.8.7 Equipment Isolation shall be the nearest isolation valve to the
equipment.

5.9 Line breaking


Line breaking is the intentional opening of a pipe, line, or other part of a process
that contains or has contained material capable of causing injury by virtue of
temperature, pressure or chemical properties to people and/or the
environment. This line breaking can be necessary for instance if blockages
prevent the normal process of draining, flushing, relieving such lines, pipes or
other process parts. In practice, this activity requires great caution to reduce the
risk of people encountering hazardous energy and/or to prevent environmental
damage. In order for this activity to be done safely, a Hazard Assessment and
control form in Work Permit (SHEM-08.10) shall be made by knowledgeable
persons with all necessary steps as indicated below shall be
considered/documented:
5.9.1 During the Hazard Assessment and Control form in Work Permits
(SHEM-08.10) developing shall include at least two person who fully
understands the applicable process and the hazards involved.
5.9.2 The reason for the line break.
5.9.3 Include the relevant LOTO Specific Procedure(s) as much as possible to
try to achieve a zero-energy state.
5.9.4 Methods to barricade the area to reduce the exposure potential while
the system is (being) opened.
5.9.5 Consider the immediate SDS control measures if something might go
wrong.
5.9.6 Test safety showers in the area and where possible have additional
running water available at the break site.
5.9.7 Whenever conditions change from the original plan, all persons
involved with the Line Break shall stop and reassess the risk.
5.9.8 Explanation of any remaining risks to the applicable persons
performing the actual line break task in the field prior to starting the
work.
5.9.9 Obtain and wear the appropriate personal protective equipment (PPE)
and ensure its condition is suitable for use.
5.9.10 Operation Supervisor to be present at first break.
5.9.11 Work out of the line of fire. Use a spray screen if necessary. Break the
flange (if applicable) away from your body by slackening the far side of
the joint first.
5.9.12 Beware of the sudden release of gases or liquids once the line is
broken.
5.9.13 Plan to collect or control any residual liquid/product and know how to
dispose of it correctly and safely.
5.9.14 Clean up any spillage immediately.
5.9.15 All isolated points / locations to be positively identified and verified by
all the stockholders
5.9.16 Assign a qualified operator to be standby during line break to act in
case of emergency with availability of the LOTO box and owner lock
keys in working area.

5.9.17 In case the operator leave the working area, the line break executers
shall stop the job till he return back.
