1. Purpose:
1.1 To ensure the isolation of power from the equipment before the maintenance
crew work on that for maintenance.
2. Scope:

5. Description:
No person is allowed to do maintenance and repairs on live equipment. LOCK-OUT-TAG-

OUT-TRY-OUT must be performed to carry out the necessary maintenance activities.
The following details describe the electrical Isolation procedure:

(Ensure "first off and Last on" process)
a) If equipment needs maintenance/shutdown, then Permit Applicant should

inform to Operation Shift In-charge and approval for Permit-to-work is needed
from them along with “LOCK OUT TAG OUT CERTIFICATE” number mentioned
in Permit to work.
b) After approval for Permit-to-work, Permit applicant need to approach Shift In-
charge (E&I Department) for LOCK-OUT-TAG-OUT-TRY-OUT process.
c) Authorised person defined by HOD(E&I) for LOCK-OUT-TAG-OUT-TRY-OUT
from E&I Department will carry out the LOCK-OUT-TAG-OUT-TRY-OUT process

by ensuring the Power & control isolation. Feeder isolation with Lock, Hasp,
and tag to be ensured with YELLOW colour lock (Equipment lock) and Key need
to be handed over to Shift In-charge (E&I Department) for authorising the
“LOCK OUT TAG OUT CERTIFICATE”. In Tag all the details would be mentioned
including the equipment number, name of permit acceptor, department name,
Date, Time, Method of isolation, Lock No, Loto box number, Isolation done by,
permit number, Isolation done by the person’s name, signature, mobile
number, and Signature of the isolation authority (E&I Shift In charge).
d) Shift In-charge (E&I Department) will hand over the isolation Key inside the
group LOTO box to the Permit applicant.
e) “LOCK OUT TAG OUT CERTIFICATE” to be authorised by Shift In-charge (E&I
Department) and Loto key number & loto box number would be entered in the
certificate and RECEIPT OF CERTIFICATE BY PERSON section is to be signed by
Permit applicant.
f) After receiving the Group LOTO box the permit holder/applicant should take
the LOTO box to operation shift in charge/permit authorizer.
g) Permit authorizer will put BLACK colour lock (Master lock) and the key of the
same would be retained by Permit authorizer till permit Is not returned and
then applicant will take the Group LOTO box at site near equipment.
h) Personal locks (BLUE COLOUR FOR ASSOCIATES AND RED COLOUR FOR AMNS
EMPLOYEE) to be applied on the loto box by every individual working on the
concerned equipment in the Group LOTO box. All personal keys should be
retained by everyone till they have not completed the work.
i) Before touching the concerned equipment absence of energy Isolation is to be
confirmed by following LOTOTO (LOCK OUT TAG OUT TRY OUT) procedure. The
applicant/holder would be checking the same from Local push button panel
installed in the field near by the equipment and would cross check the same
from Control room too.
The following SOP describe the electrical Isolation procedure if more than one
agency/department is involved in the Same equipment:
a) If there is more than one applicant for the same equipment, then all the concerned
department/agency applicant must take individual permit to work authorization
from the operation shift in charge along with LOCK OUT TAG OUT certificate number
mention in permit to work.
b) After approval for Permit-to-work, Permit applicant need to approach Shift In-charge
(E&I Department) for LOCK-OUT-TAG-OUT-TRY-OUT process.
c) Authorised person defined by HOD(E&I) for LOCK-OUT-TAG-OUT-TRY-OUT from E&I
Department will carry out the LOCK-OUT-TAG-OUT-TRY-OUT process by ensuring the
Power & control isolation. Feeder isolation with Lock, Hasp, and tag. In this case if it

found that one agency has already applied equipment lock for then the authorized
isolation person would apply one more equipment lock (YELLOW colour) in the HASP
for the same equipment with concerned agency details mentioned in respective
TAG.
d) Shift In-charge (E&I Department) will hand over the isolation Key to the applicant.
e) “LOCK OUT TAG OUT CERTIFICATE” to be authorised by Shift In-charge (E&I
Department) and Loto key number would be entered in the certificate and RECEIPT
OF CERTIFICATE BY PERSON section is to be signed by Permit applicant.
f) Then the permit applicant will inform the other agency who has already taken the
permit and working on the said equipment. Then the permit applicant of other
agencies will put the equipment lock inside the GROUP LOTO BOX already present
the site and all the other agencies person will apply BLUE colour personal locks /RED
colour personal locks in the same Group Loto box and the personal locks key would
be retained by respective owner of the personal locks.
g) Suppose one agency has completed the work then the concerned agency people
would remove their personal locks, but De-Isolation process can’t be done until and
unless all the person associated with the equipment of other agencies have removed
their personal locks.
h) If there are more people involved then more than one Group Isolation boxes are used:
When the situation demands for use of more than one Group isolation box for a particular
job (suppose two Group isolation box A and B), both the Group isolation boxes should be
interlinked by attaching an equipment lock on one box (say box-A) and key of this equipment
lock is then secured in a compartment on other Group Isolation Box-B. The Group Isolation
Box-B should be locked by Group lock. The advantage of this is that Group Isolation Box-B
can treat Group Isolation Box-A as one isolator and Box-A cannot be deactivated until the
equipment lock from Box-B has been removed.
The following details describe the electrical De-isolation procedure:
a) Permit Applicant will approach Operation Shift In-charge to close the Permit-to-
work.
b) Closure of the Permit-to-work will be done by ensuring signature of Permit Holder,
Permit Applicant & Operation Shift In-charge (Permit-to-work Authoriser). Master
lock to be removed (Black colour)
c) Permit Applicant will approach Shift In-charge (E&I Department) for electrical De-
isolation procedure.
d) Permit Applicant will return the KEY & signed copy to be ensured on COMPLETION
OF WORK at “LOCK OUT TAG OUT CERTIFICATE” (i.e., I hereby confirm that the task
is completed, the personnel & equipment are withdrawn, and the isolation may now
be removed by a competent person).
e) Shift In-charge (E&I Department) will start the electrical De-isolation procedure by
handing over the LOTOTO KEY to the Authorised person for LOCK-OUT-TAG-OUT-
TRY-OUT from E&I Department.
f) Authorised person for LOCK-OUT-TAG-OUT-TRY-OUT will remove the LOCK & TAG
and will make the feeder healthy.
g) Confirmation of DE-ISOLATION to be duly singed by Shift In-charge (E&I
Department).
Control Stop Isolation:

Control Stop is a method of stopping Equipment (where full isolation of energy is impractical to
implement) for the performance of short duration, low risk routine tasks. A separate Control
Stop procedure is developed. Where Control Stop can be used in case of the following criteria:
a) Routine Task.
b) short duration task i.e. < or = 30 Minutes.
c) Low level of Consequences i.e. Even if isolation fails it should be restricted to minor injury.
d) Control Stop Procedure should be approved by Head of Operation and Head/ Sr. Manager
Safety.
e) The Operator and working agency should be trained on Control Stop procedure.
Where Control Stop can NOT be used: Control Stop System shall not be applied to, nor
exemptions granted for the following job which are:
a) Non-Routine job.
b) Involve Electrical work on equipment or near bare conductors.
c) Involve specialist isolation like High voltage or Combustion.

2 PPE's RECOMMENDED ARE BASED ON NORMAL ACTIVITIES BEING CARRIED OUT IN THE PLANT.
ABOVE MENTIONED PPEs ARE BASED ON THE SPECIFIC PURPOSE. IN CASE OF ANY COMBINATION OF
HAZARDS, THE SUITABLE PPEs ARE TO BE USED. 
Above mentioned mandatory PPE’s must be used during LOTO/Isolation procedure depending
upon different voltage level.
