TITLE: LOCK-OUT-TAG-OUT
1. Purpose : To prevent incidents and injuries due to unexpected energization of machinery
and equipment during service and maintenance

2. Scope : This procedure is applicable to all workplace.

3. Reference : SWP-05 Safe use of Personal Protective Equipment

4. Definition :

4.1 Lock-Out – Putting a lock on the part of the machine that controls the energy, for e.g. a circuit
breaker, switch, block or valve, etc. This prevents the machine from starting up or releasing energy
accidentally.

4.2 Tag-Out – Putting a special tag to warn people of the danger of starting up the machine. It is used
when the equipment can’t be locked out.

5. Responsibility :

5. 1 It is the responsibility of the Project Manager / Person in-charge (PIC) to ensure that this procedure
is implemented at the workplace.

5. 2 It is the responsibility of the Project Manager to identify equipment and machinery that require Lock-
Out-Tag-Out (LOTO).

5. 3 It is the responsibilities of all equipment and machinery users to follow the Lock-Out-Tag-Out
procedure.

6. Procedure :

6.1 Equipment and Machinery Requiring LOTO

6.1.1 The Project Manager shall identify a list of equipment and machinery that require Lock-
Out-Tag-Out.

6.1.2 LOTO may be required for equipment and machinery that runs on electricity, hydraulic
energy (water or other liquid that moves through pipes or hoses), pneumatic energy
(pressurized steam or gases or compressed air) or mechanical energy (stored or built-up
energy in springs).

TITLE: LOCK-OUT-TAG-OUT
6.2 Only qualified and trained personnel (whether employees or appointed sub-contractors) shall carry
out LOTO and service or maintenance of machinery and equipment.

6.3 The authorized employee or sub-contractors shall follow the following steps to LOTO:

 Know the machine’s energy type, amount, and hazards, plus how to control and lock it out.
 Notify all affected personnel (including workers and subcontractors) that equipment will be
shut down and locked out for service and maintenance.
 Turn off the machine or equipment.
 Isolate the energy – the circuit breaker, disconnect switch, or other device that provides
energy to the machine or equipment.
 Lockout and/ or tag-out the energy control switches in an ‘off’ or ‘safe’ position to prevent
accidental start-up or energy release.
 Release (drain) or block stored energy, for e.g. venting pneumatic lines, draining or bleeding
hydraulic lines, block or brace any gravity-driven moving parts.
 Test the operating controls, for e.g. try to turn on all controls to make sure that no power
comes on and the equipment can’t operate, use a voltmeter or other testing device to make
sure there’s no power to the equipment.
 Remember to turn the controls back to the ‘off’ or ‘safe’ position after performing the test.
 Now, it should be safe to perform the necessary service or maintenance. Another authorized
person or party may do this.

6.4 Steps to Restart Equipment or Machinery

Once the maintenance or repair work is completed on the machine, follow the following steps to
restart:

 Inspect the equipment or machinery to be sure all tools and other materials have been
removed, the machine is fully re-assembled, and that guards and other safety features are in
place.
 Notify all relevant personnel (employees as well as sub-contractors) that equipment will be
starting.
 Remove all lock-out devices. Remember that only the person who put the lock may remove it.

6. 5 Type of Locks

6.5.1 The Project Manager shall identify the type of locks approved for LOTO. These locks
shall be durable to withstand heat, cold, humidity or corrosiveness, standardized in color,

shape or size throughout the workplace, strong (can’t be removed without heavy force or
tools) and identified by the name of the authorised personnel who installs and removes it.

6. 6 Type of Tags

6.6.1 The Project Managers shall identify appropriate tags to be used for tag-outs. These tags
shall have the same print and format throughout the workplace, be easy to read and
understand, tough enough so they can’t be removed accidentally. The ties for tag-outs
shall be self-locking, can’t be re-used, and can be attached by hand (e.g. made of nylon).

6.6.2 Tags shall display warnings such as “DO NOT START”, “DO NOT OPEN” or “DO NOT
OPERATE”.

LOTO REMINDERS

1 ALWAYS lock or tag machine energy sources before maintenance or repair.

2 USE lock-out whenever possible. Tag-out is just a warning, not an active way to prevent
accidents.

3 FOLLOW all lock-out/ tag-out installation and removal steps.

4 BE SURE other employees in the area are aware of your lock-out/ tag-out activities.

5 NEVER ignore someone else’s lock or tag.
