5. Responsibility :
5.1 The Project Manager (PM)/ Person in-charge (PIC) is overall responsible for proper usage and
maintenance of all lifting equipment, appliances and gears used at the workplace.
5.2 The PM/ PIC and Supervisor are responsible for ensuring proper lifting operation control of all lifting
activities.
5.3 The PM/ PIC is responsible for regular monitoring of the legal requirements.
5.4 The crane operator is responsible for maintenance of the daily checklist.
6. Procedure :
6.1 Lifting Equipment and Appliance i.e. Gantry Crane, Pulley Block, Chain Block, etc.
Through regular inspection, the PIC and Supervisor shall ensure that:-
No lifting equipment and appliance is being loaded beyond the safe working load except by an
Authorised Examiner or an inspector for the purpose of testing such equipment and appliance.
Every lifting equipment and appliance is clearly marked with its safe working load.
Every lifting equipment and appliance is being thoroughly examined at least once in every 12
months by an Authorised Examiner.
All relevant reports for tests conducted must be kept at the factory for legal requirements/
compliance verification.
Through regular inspection, the PIC and Supervisor shall ensure that:-

The Project Manager/ PIC and Supervisor in-charge shall ensure that the following safety
measures are implemented at the workplace where lorry cranes are used:
All outriggers of the lorry crane shall be fully extended irregardless of the weight of the load to
be lifted and the distance where the load is to be lifted or unloaded.
All outriggers of the lorry crane shall be set on firm ground and the ground shall be level as far
as reasonably practicable.
Lorry crane shall be operated by trained and experience operator only.
The safe working load of the lorry crane and the lifting gears shall not be exceeded during
lifting operations.
Lifting supervisor and riggers shall check and ensure that the lifting gears used are in good
condition and is appropriate for the load to be lifted.
Rigging of the load shall be carried out by qualified riggers only.
Lorry crane shall not be used for pulling or dragging of load.
Where the lorry crane is to be operated on public roads and/or pedestrian walkways,
appropriate LTA approved traffic diversion and safety signs and safety devices (e.g. blinker
lights, cones, etc.) shall be installed to warn other motorist/ pedestrians.
6.4 During operation the PM/ PIC/ Lifting Supervisor shall ensure that no crane is being loaded beyond
the safe working load.
6.5 The PM/ PIC and Lifting Supervisor shall ensure that every crane including all shackles, sheaves,
wire rope and the various devices have been inspected as per WSHA regulation.
Checklist for Testing and Inspection by Approved Person and Certificate of approved
person at least once a year.
6.7 Non-conformance
6.7.2 The PM/ PIC shall take appropriate actions against the responsible parties and shall
ensure that the rectification work has been done accordingly.

1. LOAD
1.1 Has the actual weight of the load inclusive of the rigging accessories been established?
1.2 Is the established load within the safe working load of the lifting equipment?
1.3 Is the load rigged to its centre of gravity?
1.4 Is a tag line provided to control movement of the load?
1.5 Is the load being lifted in a level and stable manner?
2. LIFTING EQUIPMENT
2.1 Is the selected lifting equipment fit for purpose and appropriate for the lifting operation?
2.2 Has the lifting equipment been examined at least once in every 12mths (or 6mths for those
lifting personnel) by an A.E. and does it possess a valid lifting equipment certificate?
2.3 Has the lifting equipment been marked conspicuously with the SWL which it can safely carry?
3. LIFTING OPERATION
3.1 Is there a lifting plan available for the operation?
3.2 Has risk assessment been conducted to ensure that all possible hazards are eliminated and
risks are reduced?
3.3 Has the identified control measure(s) been implemented?
3.4 Is the area for lifting the load and the lifting path been cleared of obstruction and personnel?
3.5 Has the designated area for placing the load been cleared or properly prepared prior to the
lifting operation?
3.6 Is the load likely to clash with other structures or equipment along its lifting path?
3.7 Has a safe means of access or egress been provided for personnel to attach or recover the
rigging of the load?
3.8 Has the means of communication during the lift been established and confirmed?
3.9 Has the lifting zone been barricaded to prevent access by unauthorised Personnel?
4. Personnel involved in lifting operation, such as equipment operator, signalman, rigger and
lifting supervisor
4.1 Have trained and competent lifting personnel been identified for lifting operation?
4.2 Have all lifting personnel such as operator, lifting supervisor, signalman and rigger been
briefed at a tool box or pre-lift meeting?
