1 Purpose
1.1 To prevent unauthorized work in confined spaces
1.2 To ensure employee safety and prevent serious injury or fatality in the workplace

2 Training Requirements
The following employees shall be trained on this guideline:

2.1 Production and Plant Manager, Supervisors, Foreman, Operators, Helpers, and Staff
2.2 Maintenance Manager, Supervisors, Technicians, Helpers, and Staff
2.3 Services Manager, Supervisors, Foreman, Mechanics, Electricians, and Staff
2.4 Fabrication Manager, Supervisors, Foreman, Mechanics, Electricians, and Staff
2.5 Materials and Logistic Manager, Supervisors, Coordinators, Foreman, and Drivers
2.6 Technical Manager, QA/QC Engineers, Laboratory Supervisors, Foreman, and
Technicians
2.7 QHSE Manager, HSE Officers, and Security Officers

3 Guidelines

3.1 Before the Activity


3.1.1 Inform the HSE Officer of the task requiring confined space entry.
3.1.2 Request for the Confined Space Entry Permit from the HSE Officer and follow the
applicable precautions listed in the permit, i.e.:
i. Notify all concerned personnel of the planned activity
ii. Identify the personnel to enter the confined space and the designated stand-by
(buddy) outside
iii. Provide the necessary PPE and safety equipment, e.g. safety harness with lifeline
at all entrances
iv. Ensure proper access to the confined space is available
v. Check for sufficiency of ventilation
vi. Test for oxygen deficiency
vii. Check if any toxic gas is anticipated
viii. Arrange for low voltage and flameproof lighting

ix. If applicable, check if lock-out tag-out (LOTO) is implemented – Refer to GCSI-HSE-


OPC-004
x. If applicable, verify that the Pre-Task Checklist for cleaning central mixer is
completed
xi. Determine any other necessary precautions based on the nature of the activity
3.1.3 Seek approval from the immediate superior then submit the filled-out permit to the HSE
Officer.
3.1.4 The HSE Officer shall verify that the above precautions are done. If approved, the name
or badge nos. of the personnel involved shall be included in the form. Otherwise, the
reason for declining the request shall be specified.
3.1.5 Upon approval, request for a copy of the form to be made available in the designated
work area.
3.1.6 Barricade the work area and place warning signs before commencing work.
3.1.7 Ensure that the work area is away from heat ignition sources or activities, e.g. smoking,
open lights, torch, flames, etc.
3.1.8 In case welding or other hot works is required inside the confined space, secure the
Daily Hot Work Permit and follow GCSI-HSE-OPC-006 Welding and Hot Works.
3.1.9 In case the confined space is at 1.8m or above, secure the Working at Height Permit and
follow GCSI-HSE-OPC-003 Working at Height.

3.2 During the Activity


3.2.1 Ensure that the stand-by (buddy) has reasonable communication method with the
personnel inside the confined space. Do not leave the work area without replacement.
3.2.2 The stand-by (buddy) shall check on the personnel inside the enclosed space from time
to time.
3.2.3 Consult with the HSE Officer as often as necessary to ensure safety while performing the
activity.

3.3 After the Activity


3.4 Check that all personnel authorized to go inside the confined space has come out.
3.5 Return the man-hole / grill cover properly.
3.6 Clear the work area, remove any used tools, and ensure that it is left safe and free from
hazards.
3.7 Inform the HSE Officer once the activity is completed. The HSE Officer shall verify the
above and note these in the permit.
3.8 In case of an incident or accident, the Incident Investigation Report reference no. shall
be included in the permit.
