PART 5 (General Guidelines and Terms & Conditions for the completion and issuances of ID
Cards)
1. Applications shall be submitted to Pass Administration Department
for the approval at least 24 hours in advance.
2. Application forms must be accompanied with a copy of passport &
QID duly attested by the employer’s authorized representative.
3. All Contractors are responsible for safe custody of ID Cards issued to
their employees.
4. All expired ID Cards shall be returned to the Pass Administration
Department.
5. Loss of ID Cards shall be thoroughly investigated by the end-users
and lost report forwarded to GCSI Pass Administration Department within 24 hours after the incident.
6. Any damaged or mutilated ID Cards are to be returned to the Pass
Administration Department for replacement. Company
7. Php 100 per ID card will be levied against individual company for Stamp
any lost, damaged or mutilated ID Cards.
