3. QA Concrete testing i.e. slump test is not being conducted. Time of arrival of concrete at site must be
validated against the batching time. Upon arrival, the slump and temp must be checked immediately. This
is crucial especially if the concrete placement is at higher levels using concrete pump where slump lost is
susceptible.

4. No proper consolidation method of concrete being done during placement

5. No concrete cover blocks placed to ensure sufficient concrete cover is achieved during concreting

o be randomly done to ensure minimum of 85 microns is achieved as per BS EN ISO 1461/ASTM A 153.

Mottling Oxidation/White Rust Dross Inclusions

8. Missing bolts (Quality & Safety Non-compliance). Verify the termination between the face of the wall/slab
with the bracket. No notching/cut-out was done.

single scree as shown in the picture.

D. SAFETY OBSERVATIONS:

1. Refer to No. 8 and No. 9 in the above Quality Observations – Major Risk

2. Concrete Pump (boom pipe and hose) – Lack of Isolation Measures
