4. Missing bolts (Quality & Safety Non-compliance). Verify the termination between the face of the wall/slab
with the bracket. No notching/cut-out was done. – Update: Not Resolved

5. Unsafe fixation of the mullion to the bracket. Some mullions don’t have any fixation and some only have a
single scree as shown in the picture. – Update: Not Resolved

E. SAFETY OBSERVATIONS:

1. Refer to No. 4 and No. 5 in the above Quality Observations – Major Risk
2. Leaving load on the cantilever loading platform. No valid 3rd independent certificates displayed showing the
design SWL capacity. The inspection report/receipt received from 3rd party doesn’t mention in effect the safe
/fit to purpose status of the CLP and have not mentioned the safe working load to guarantee safe working
order.
