WEEKEND LECTURE – TENS & IFC

OVERVIEW

SETTINGS FOR PAIN

CONVENTIONAL ACUPUNCTURE
Long Duration (up to 24 hrs) 20-30 minutes
Strong, but comfortable Strong visible twitch
tingling Electrodes on muscle bellies
 Pulse Duration (50-80 μs)  Pulse Duration (150-300
 Frequency (100-150 Hz) μs)
 Gate Control Mechanism  Frequency (2-10 Hz)
 Immediate, but short-lived  Endo-opioid Mechanism
analgesia  Analgesia up to 5 hrs
Small Region Large Region
TENS Only IFC or TENS

- Pain-relief in conventional is gone after removing the

electrodes, acupuncture lasts longer
- Start with the lowest Pulse duration but highest frequency
(e.g., 50μs and 150Hz)
- Acupuncture can elicit discomfort so inform patient

APPLICATION
TENS - Stimulation of A fibers branches to inhibitory interneuron
- Can use 2 or 4 and activates it
- The arrangement doesn’t matter as long as there is at least 2 - If A fibers stimulation is greater than A, it will have net
(Black-Black, Red-Red, Red-Black) activation of Inhibitory Interneuron to close the gate
- Can be placed faraway (2 on lumbar 2 on scapula)
ENDO-OPIOID MECHANISM
IFC
- Use all 4
- Has to crisscross diagonally (Black-Red-Black-Red)

GATE THEORY

Pain Perception
- Pain is perceived in the brain
- First-order neuron enters in dorsal root ganglion > Synapse
with Second-order neuron in SC > Opposite of SC goes up
to Thalamus > Third-order neuron carries information to
the brain
- Nociceptor (First-order Sensory Neuron unipolar) > Axon
leads to cell body of DRG > goes to spinal cord and
synapse with cell body of Second-order neuron (+)

- Unipolar Neuron (Sensory)/First-order neuron: pick up Pain Inhibition

information when you cut your arm - Midbrain: PAG
- Axon of 1st order Neuron synapse (+/activates) with cell Medulla: NRM
body of 2nd order neuron - These regions of the brainstem activates and release
- 2nd order neuron carry information to thalamus which Serotonin that activates descending tract to inhibit pain
synapses with 2nd order neuron (Inhibitory Interneuron)
- Axon in first-order neuron to inhibit (-) the inhibitory - Also releases 3 Opioids (-endorphin, Met- & Leu-
interneuron and can no longer inhibit second-order neuron enkephalins, Dynorphins
- Summary: Direct stimulation of second-order neuron and
inhibition of inhibitory interneuron
- First-order neuron = A or C fibers
To Reactivate Inhibitory Neuron to Close Gate
