Re: Conduct of Triage Activity to all PDL

1. Reference: Memo from JCSUPT INGANCIO S PANTI, Regional Director,

NCR, dated 27 April 2020 re Conduct of Triage to all PDL.

2. ICOW the above reference, be informed that this facility conducted triage
to all Six hundred eighty eight (688) Persons Deprived of Liberty (PDL) OOA 15 1430H
August, 2020 held at the 3rd floor area by the duty nurse, SJO2 Tito A Galla, RN, using
the Case Investigation Form (CIF) from the Department of Health (DOH) and COVID-19
Symptom Surveillance Form from the Bureau.

3. Further, be informed that no PDL suffered from fever, cough and other
symptoms related to Covid-19.
