“Sampong beses na po
akung sumaka sa
isang araw” as
verbalized by the
patient.
Cannot manage to
tolerate any food for 3
days.
Feels very weak.
Abdominal pain that
is constant, sharp, and
burning.
Objective:
Vomiting
VS as follows:
- HR: 96 bpm
-Lying BP: 115/68
mmHg
-Standing BP: 98/55
ACTION
Dx:
Determine causes of
nausea.
Evaluate the patient’s
response to antiemetic
or interventions to
alleviate the
condition.
Assess patient for the
degree of vomiting:
mild, moderate, or
severe.
Tx:
Provide mouth care
and emesis basin
within the reach of the
guardian.
Assist and encourage
the patient in upright
position while eating
and for 1 to 2 hours’
post-meal.
Administer
antietemics as
ordered.
Edx:
Educate the patient
about the importance
of changing positions
slowly and calmly.
Encourage oral
hydration; Administer
oral rehydration
solution as necessary
or IV fluids as
appropriate.
Instruct the patient to
take prescribed
medications as
ordered.
