AFFIDAVIT

I, MARIVIC ONGPAUCO, of legal age, and with address at Blk 53 Lt 1 Escopa

II, Quezon City, after having duly sworn to in accordance with law, herby depose
and state that:

1. I am the live-in partner and legal guardian of Lincoln Caguindagan

Malarde, an SAP beneficiary;

2. I am authorized to execute this affidavit by virtue of the letter of

authorization executed by Lincoln Caguindagan Malarde attached hereto as Annex
A;

3. The SIM card he registered to receive SAP benefits is 09480975011

(the “first SIM card”);

4. The first SIM card is no longer active and Lincoln Cagundagan

Malarde is now using the SIM card registered under the number 09668813988 (the
“second SIM card”);

5. However, Eight Thousand Pesos (Php 8,000.00) was already

transferred through the first SIM card and can no longer be recovered;

6. I authorize the cancellation of the transfer to the first SIM Card and
the transfer of the same to the second SIM Card.

7. I execute this affidavit to attest to the truth of the foregoing.

Further, affiant sayeth naught.
