Work Procedure Checklist
1. Pre-Operation: -
Must do inspection of the truck prior to start work
Check the tail gate for locking and unlocking mechanism
Check the tire in good condition; also check the brake system
Wear all the mandatory and additional PPE required as above
Ensure reverse alarm; lamps (especially for night work) in working condition
Check with APM supervisor the correct tipping area and hazards present at this specific area
Now the correct and safe route to transport the clinker

2. Operation
Access into the loading areas (export silo \ off-std silo) via the correct access
Have good visibility, can see the chutes, operators and also the access path
Look for pedestrian nearby (honk) to alert them
Position the truck as the operator unload the clinker into truck bed, wait for the operator
instruction.
Ensure the tail gate locked and no spill of material from the truck bed
Move out after the telescopic chute have fully retracted from the truck height and move to
loading area toward the tipping area (bays), watch out for other trucks; use the perimeter
road to minimize contact with pedestrian
Minimize need to enter the bay in reverse; if need flagman to assist inform APM supervisor
Honk before entering the bay in reverse; park the vehicle on level surface
Check the tailgate unlocking system, check no obstruction during dumping; then dump the
load
Move out from the loading bay safely
3. Post-Operation
Park the vehicle at designated area informs by APM supervisor
Ensure place wheel choke at truck tire
Only perform any maintenance work at garage
Ensure any oil spill \ hydraulic leaks clean
Hazard Control
Struck by other vehicle Obey plant speed limit (15 km/h); provide room for other heavy
machinery or plant; give priority for trailer \ containers; minimize
reverse
Vehicle rollover Only dump on level surface; ensure tail gate in good condition; make
sure all the lifting cylinders in good condition
Overhead cable \ structure Never tip material nearby any overhead power cable, safe distance of
at least 10 m from the overhead power cable; also check for the bay
heights before tip, to avoid bed stuck at the structure
Slip, Trip, Fall Use 3-point contact while climb up or down from the truck cabin
Fall from heigh Avoid to climb up the bed without adequate means of fall protection
(life line or safety harness)
