PARENT CONSENT FORM

I , parent of of class & sec hereby

give my consent and have NO OBJECTION to my ward coming to school physically to attend
offline classes.

I further declare that:

i) I understand the risks of COVID-19 transmission which exists in the present
situation.
ii) My ward will wear mask and I will sensitize her/him to maintain social distancing,
sanitize her/his hands from time to time, follow COVID Appropriate Behaviour.
iii) I will instruct my ward not to share books, note-books , stationery items, tiffin box
etc.
iii) My ward is maintaining good health and is not suffering from any illness at the
moment.
iv) I will not send my ward to school if she/he is unwell or displays any symptoms
- cough, fever, difficulty in breathing and bodily weakness.
v) I will immediately inform the school authority if any symptom appears in my ward.
vi) I am also aware that the school will take all precautions and will follow the COVID
19 protocol, however, I will not hold the school responsible in case she/he
contracts COVID19 once she/he leaves home.
vii) I can be contacted on at all times, in case of any
emergency.
